I was advised that one of the forms I completed on Zocdoc was never received. I advised the gentleman (Chase) that I really didn’t feel like completing the form again. I was then told, “here it is.” I’m not sure it was actually in their possession. Chase was polite but as he assisted in my examination and cleaning services he was chewing gum under his mask. Since he was wearing a mask at the front desk, I can only hope that he put on a new mask before assisting the dentist. Also, it’s the first time that I ever had my teeth cleaned and didn’t get flossed. I always assume that was part of the cleaning.
