How to submit a claim if you have a UnitedHealthcare health plan
You may not need to submit a claim for medical services provided to you if you consult an in-network provider in most cases. UnitedHealthcare will pay for covered health services. Prior authorization means you must get approval from your health plan provider before accessing a particular medical service or medication. With prior approval, your health plan agrees to help cover the cost of the service, subject to any cost-sharing or other limitations.
