What is your typical process for working with clients?
You will receive two emails: one to set up a profile with our billing company and another email that includes electronic medical intake forms. Please fill these out on a computer or a laptop.
Please fill out the intake forms as soon as possible after booking your appointment. The forms are important and helps me to set up your patient profile for my office system. I ask that you please check your email and text messages.
