Getting to know Alexander Peguero-Medrano

I am a board-certified family psychiatric nurse practitioner with experience in psychiatry since 2012, beginning my career as a registered nurse. I earned my master's degree in psychiatric nursing from Hunter College, City University of New York, in 2017. I have worked in acute care settings as an emergency room nurse practitioner providing psychiatric consultation services, as well as in outpatient clinics caring for individuals of all age groups. My focus is on medication management while offering psychoeducation, support, and empathetic understanding of your needs.

What should a new client know about working with you?

I offer a comprehensive approach to understand your needs and provide resources as part of psycho-education. I only offer medications if we agree together it may be beneficial or you have failed alternative forms of treatment like psychotherapy. I would not prescribe without informing you of the risk vs benefits of treatment. If you’re prescribed medications, I can either continue the form of treatment or help you manage your medications if there are any changes with your mental well-being. Prior to our initial session, I would like to speak briefly about your needs by phone and can speak with you for up to 15 minutes at no extra cost.

What is your typical process for working with clients?

Initial session involves talking about past and current symptoms. I believe that symptoms are multi factorial and can be exacerbated by many stressors in life. Follow up sessions involve managing medications if they’re prescribed and discussing any new or persistent symptoms. I offer brief therapy at subsequent follow-up sessions and often recommend some form of psychotherapy with another provider.

Dr. Peguero-Medrano is truly the best psychiatrist I have ever worked with. He works hard to find solutions that work for you and isn’t so medication-oriented. He leaves it as a choice to the patient rather than the only path of discussion. I am so grateful I found him!
Alex Medrano is knowledgeable, asks the right questions to ensure the best method of care, patient, calming, supportive, accommodating and listens intently. A true professional. I have been seeing Dr. Alexander for a close to a year now and having him as a doctor, I never feel pressured. It’s like talk to an old friend he is awesome !
My visits with Alex are always a great experience. He is very accommodating, always on time, extremely understanding and professional. He has been a lifesaver in my maintenance and recovery, and after seeing more psychiatrists then I can count, I finally found the one who has not given up on me.
Alex is diligent and reasonable while also being caring and understanding. At the same time Alex is blunt and will go through every procedure possible to cross out any possible diagnosis before officially finding the one that effects you. You will be pushed but for the better and if you aren't ready to help yourself it will be harder for both parties! I reccomend Alex if you want genuine help and I'd reccomend him even more if you didn't have to go through stony brook system besides that hes a gem !
