This involves regular screenings, personalized treatment plans, and coordinated referrals to mental health specialists as necessary, ensuring that both aspects of health are treated in tandem for optimal patient outcomes. By fostering a supportive environment, the PCP can help patients manage chronic conditions, prevent diseases, and promote overall well-being. Tell us a little bit about yourself. What do you like to do in your spare time? I enjoy activities that inspire and promote well-being, including time with family and friends. I have a great love of the outdoors, reading, music and learning about new things. You might find me engaging in activities such as yoga and mindfullness; listening to science or health related podcasts; splashing in puddles with my toddler; singing, playing and writing music; all that brings me joy.
