Thank you for visiting my profile. Your illness does not define you—your strength and courage do. Now is the time to make your mental health a priority. I am a board-certified Psychiatric and Mental Health Nurse Practitioner dedicated to providing individualized care to people across the lifespan. My professional role involves caring for some of society’s most vulnerable citizens, with experience in diverse clinical settings. Originally from New York and now residing in Georgia, my vision is to make mental health care more accessible to everyone. I treat patients of all ages and have expertise in managing anxiety, depression, grief, life transitions, medication management, obsessive-compulsive disorder, relationship issues, self-esteem challenges, sleep disorders, stress, trauma, PTSD, impulse control disorders, and mood disorders. I hold a Doctor of Nursing Practice from Walden University and a Master’s degree in Psychiatric and Mental Health Nursing from Valdosta State University. Outside of work, I am a mother of four and an Army veteran. I enjoy reading, cooking, crocheting, spending time with my family, and watching football.

More information on payments and coverage

Payment is due after each session and will be charged with your card on file for any portion of patient responsibility. Receipts will be provided at the time of the charge. This typically takes place 2-4 weeks after the session, once your insurance has had a chance to process the claim. We try our best to verify eligibility, but it is ultimately your responsibility to check coverage and out-of-pocket costs for services. For out-of-network visits, you will be billed at the provider’s self-pay rate, and we can provide an invoice for you to submit to your insurance for potential out-of-network reimbursement based on your plan’s benefits.

She would regularly claim that medications I received from the Kroger pharmacy did not match what she prescribed me. I thought this was a pharmacy error - type (instant or delayed release) or written instructions on the bottle about dose schedule (1x, 2x, 3x per day). 1st red flag - she became convinced that I was seeing another provider. I showed the bottle with "Prescriber: Amber Dale" over webcam. On the next visit, she claimed that I previously "admitted" to having a 2nd doctor. 2nd red flag - she later claimed to intentionally not refill a mental illness medication (previous visit she said to switch from XL to SR type). She never informed me to stop taking it, and never ramped me down to stop. It was part of the "2nd doctor" ordeal. The pharmacy texted me for a refill. It made me lose my faith in my safety under her care. She _doubled_down_, it felt dangerous considering that she was both guiding my medications and unaware of what I physically was receiving from the pharmacy.

Amber seemed like she was listening at first, but as I explained some of the harder issues I was having, it felt as though she was down playing them. When I told her about a very specific thing that happens when I’m overwhelmed, she almost mocked me and said, “Wait you do WHAT?!” and pretty much laughed at me. When I saw another provider, that very key thing led to more questions and scenarios that led to me getting diagnosed with Borderline Personality Disorder. I felt as though Amber had other places to be or things to be doing and that I was just bothering her. If you already have a diagnosis I may recommend her, but not if you’re looking to figure out why you are the way you are or have anything that needs a deeper look.

Dr lacks bedside manners. She appeared short tempered, & as if she just wanted the appointment to be over. There was no formal introduction to ease into things, & she seemed to lack genuine interest in what I had to say.
