Getting to know Amy Burchett
Amy began her career in the medical field in 2003 as a Surgical Technician. She advanced her education and earned her Registered Nurse (RN) designation in 2007. Demonstrating a commitment to lifelong learning, Amy furthered her studies and became a Nurse Practitioner specializing in Family Medicine in 2021. Recognizing a significant gap in mental health care options for her patients, she obtained her Psychiatric Mental Health degree to enhance the services she offers. In 2024, Amy transitioned to working in substance use and mental health care management. Her vast background in nursing and Advanced Practice Nursing brings a unique combination of skills and expertise to our practice. She is dedicated to delivering compassionate and comprehensive care to our patients.

Once you have booked an appointment with Mind Body Health & Wellness, LLC, you will receive a patient portal invite. Here you will find all intake forms to complete. You will be required to keep a credit card on file. All appointments will require payment prior to the appointment you will be directed to provide at your appointment time.
Any missed appointments will result in a No Show fee of $150.
Initial Evaluation - 60 min $275
New patient visits are the first step to managing your care. We will review your current medications, sources of care, and any concerns you may have. We also collect detailed medical, family, social, and psychiatric history. This includes hospitalization and medication history. Come prepared with this background!
Follow-up 30 min $130
Effective mental health management requires ongoing evaluation and adjustment. A 30-minute follow-up is suitable for assessing the effectiveness of medications and making minor adjustments to the medication plan.
Follow-up 60 min $220
Monitoring medication and ensuring quality care are essential for maintaining long-term health and wellness. A 60-minute follow-up session is recommended when there is a decline in mental health, urgent situations arise, medications become ineffective, significant life changes occur, or when education on coping and stress management techniques are requested.
Ten-minute phone/email consultation $50
Between scheduled appointments for established patients only
Professional Documentation Services $50
Completion of forms; copy of records; and/or additional time spent on other professional services. (Please make note that a client must have a minimum of six prior appointments for Mind Body Health & Wellness, LLC to render legal, FMLA, or other detailed documentation services.)
