Getting to know Carolyne Mburu-Gerena

I am a board-certified Psychiatric Mental Health Nurse Practitioner providing psychiatric evaluations and medication management for adults experiencing anxiety, depression, ADHD, mood disorders, and stress-related mental health concerns. My approach is structured, collaborative, and focused on accurate diagnosis, thoughtful medication planning, and ongoing symptom improvement. Appointments are centered on understanding your clinical history, current symptoms, and treatment goals in order to develop a medication plan that is both effective and well-tolerated. I emphasize education, realistic expectations, and shared decision-making so patients feel informed and supported throughout care. I work frequently with individuals who have tried medications in the past with limited success, experienced side effects, or feel uncertain about next steps. Follow-up visits focus on monitoring response, making appropriate adjustments, and supporting long-term symptom stability. I do not provide weekly psychotherapy, but I collaborate with therapists and other healthcare providers when therapy is part of a comprehensive treatment plan. Patients seeking medication-focused psychiatric care tend to be the best fit for my practice. Please note that controlled medications are not prescribed for patients located in Rhode Island, New Mexico, Vermont, or Colorado, and treatment options will be discussed based on clinical appropriateness and state regulations. All appointments are conducted via secure telehealth. To ensure a productive visit, patients must complete the intake packet within 24 hours of booking. Appointments may be rescheduled if required forms are not completed in advance.

What should a new client know about working with you?

I provide psychiatric evaluations and medication management for adults, with a focus on clear diagnosis, thoughtful medication planning, and ongoing follow-up care. Visits are structured and goal-oriented, and we work collaboratively to find treatment options that are effective and well-tolerated. My practice is medication-focused rather than therapy-based. While I value therapy and often collaborate with therapists, I do not provide weekly psychotherapy sessions. Patients who benefit most from working with me are those seeking ongoing psychiatric care and medication management. All appointments are conducted via secure telehealth. New patients are expected to complete the intake packet within 24 hours of booking so we can make the best use of our time together. Please be aware that controlled medications are not prescribed for patients located in Rhode Island, New Mexico, Vermont, or Colorado, in accordance with state-specific regulations. If you are looking for a structured, professional approach to psychiatric care with clear expectations and continuity over time, my practice may be a good fit.

What is your typical process for working with clients?

Booking & Required Intake After scheduling, new patients receive an intake packet that must be completed within 24 hours of booking. This information is required and allows for a focused, productive visit. Appointments may be rescheduled if forms are not completed in advance. Initial Psychiatric Evaluation The first visit is a comprehensive psychiatric evaluation focused on diagnosis, medication history, and current symptoms. Appointments are structured and time-limited, with an emphasis on developing a clear treatment plan rather than ongoing talk therapy. Medication-Focused Treatment Planning When appropriate, we discuss medication options, expected benefits, possible side effects, and next steps. Treatment decisions are collaborative and based on clinical appropriateness, state regulations, and patient goals. Controlled medications are not prescribed for patients located in Rhode Island, New Mexico, Vermont, or Colorado. Ongoing Follow-Up Care Follow-up visits focus on monitoring response, adjusting medications as needed, and supporting continued symptom improvement. Patients are expected to participate in recommended follow-up care for treatment to remain effective. Coordination of Care (Not Weekly Therapy) I do not provide weekly psychotherapy. Patients seeking therapy-only services or brief, one-time medication visits may not be a good fit. I do collaborate with therapists and other providers when appropriate. All appointments are conducted via secure telehealth. This process works best for patients seeking structured, medication-focused psychiatric care, who are open to ongoing follow-up and clear treatment expectations. This practice is not a good fit for those seeking therapy-only care, immediate or guaranteed stimulant prescriptions, or one-time evaluations without follow-up. Treatment is focused on thoughtful diagnosis and medication management for ADHD and anxiety, with an expectation of ongoing care and collaboration.

Dr. Carolyne was a great listener I felt like I was really understood and I got the right medication that I needed. She is very quick and very intuitive when it comes to looking at the medical paperwork for a diagnosis and previous diagnosis’s as well. She explained to me that a previous condition I had wasn’t exactly accurate and I felt like I really understood what she was talking about. She was very patient and understanding I highly recommend her.

We have had negative experiences in the past with doctors and this was an amazing reminder that there are caring and kind practitioners out there.
Video visit
She's easy to talk to and has a good sense of humor. I probably sound insane a mile a minute and she was just patient and helpful. Suggested medication, but didn't push. I have a follow up in two weeks. Psychiatry is an important component to anyone dealing with actual mental health issues, not just being "sad" once in a while. Having a medical doctor/nurse alongside a talk therapist if you want is totally necessary in understanding what issues you may have, and be properly diagnosed.

Carolyne is a wonderful provider! From my very first appointment with her, she has made me feel comfortable and at ease. She is a great listener (and funny!), empathetic, and makes me feel heard with every visit. She has always been very responsive and has gotten back to me quickly when I have reached out with any questions or concerns. She truly cares and makes it a point to schedule a follow up appointment each visit. Thanks for being a great caregiver!
