I am Chester Profeta, PMHNP-BC. I've worked in mental health for about 16 years, starting as a psychiatric nurse in hospital settings and moving into outpatient care and prescribing. A lot of my patients are students. I work with test anxiety, ADHD, and the burnout that comes with nursing school, medical school, and heavy course loads. I'm certified as an ADHD Clinical Services Provider, and I take time with the diagnostic process rather than rushing to a prescription. Medication is one tool. I also do supportive therapy in my visits, because what brings someone in is usually bigger than a chemical problem. Sleep, class schedules, family expectations, pressure they haven't said out loud to anyone. We work on all of it. Important information about controlled medications: Controlled medications, including stimulants and benzodiazepines, are not prescribed at the initial appointment. A comprehensive psych eval is required before determining whether these medications are appropriate. If you are seeking an immediate prescription or refill for a controlled medication, my practice may not be the right fit for your needs. I see patients by telehealth across California and in person at my office in downtown Almaden, San Jose. I also speak Tagalog. If you're new to psychiatric care, or you've had a bad experience with it before, I'd rather you leave a visit knowing exactly what you're taking and why than nodding along to something you didn't follow.

During appointments, I try to keep visits comfortable, straightforward, and collaborative. I’ll ask questions to understand the full picture, not just your symptoms. I also use supportive therapy and practical strategies when they can help. My goal is for you to leave knowing what we’re working on and why.

What is your typical process for working with clients?

Your first visit is a full psychiatric evaluation focused on your symptoms, history, medications, daily functioning, and treatment goals. We’ll discuss possible diagnoses and treatment options together. I want you to understand the plan and feel comfortable with it. We’ll decide on next steps based on your needs. If medication is appropriate, I’ll review the risks, benefits, and what to expect. Follow-ups focus on response, side effects, and any needed changes. I also use supportive therapy and practical strategies when helpful. ADHD evaluations may include rating scales, childhood history, family input, or prior records.
