Dr. David Oluyinka Isaac is an Advanced Registered Nurse Practitioner (ARNP) licensed in Washington State and a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC). He began his career in 2011 as a Certified Nursing Assistant, working in nursing homes before obtaining his Practical Nursing license in 2014. While working as an LPN in a maximum-security psychiatric hospital, passing medications to at least 50 patients daily, he decided to return to school and became a Registered Nurse in 2016. He continued to work in the same maximum-security psychiatric hospital while enrolled in a graduate nursing program in 2018, and he moved to the state of Washington for an outpatient PMHNP position after graduating in 2020. Dr. Isaac's doctoral work continued at the University of South Alabama, Mobile, Alabama while treating adolescents and adults with psychiatric disorders both in-person and via telehealth in outpatient and inpatient settings. His doctoral quality improvement project titled "Increasing Bowel Motility and Quality of Life Among High-Risk Clozapine Patients Through Shared Decision-Making Lifestyle Modifications and Bowel Movements Monitoring" was inspired by his belief in not only the importance of mental health, but also that of physical health and shared decision-making in enhancing best patient outcomes. He obtained his Doctor of Nursing Practice degree in December 2022. Dr. Isaac treats several mental health disorders and provides a neutral ground to individuals and families. He is a member of the American Psychiatric Nurses Association (APNA) and a lifetime member of the Sigma Theta Tau International Honour Society of Nursing and Golden Key International Honour Society. In his free time, he enjoys watching and playing sports, including soccer; he enjoys watching movies, reading, cooking, traveling, and spending time with his friends and family.

INTAKE, INSURANCE, AND PAYMENT To ensure a smooth intake process, please provide us with your complete insurance details. Surround Me Mental Health PLLC requires your valid credit information on file prior to the initial visit, which can be self-entered into your patient portal when you’re invited to do so. If you do not already have the information, please contact your insurance company regarding your copayment, deductible, coinsurance so you know if you would be responsible for zero, part or all the cost of the services provided to you. Patients are responsible for full payment, whether insurance company ends up paying partially, or not at all, for services rendered by Surround Me Mental Health PLLC. If SMMH is out-of-network, patients will pay the full cost of services out of pocket. SMMH can provide superbill that patients can submit to insurance company for reimbursement. If SMMH is in-network and you are yet to meet your deductible, you will be expected to pay the maximum payment out of pocket that the plan will pay for a covered health care service. If you’ve met your deductible, you’ll pay your coinsurance or copayment amount instead. All payments are due at the time of service. To streamline the intake process, you will receive and return the required completed intake paperwork via your patient portal. Instructions to create your patient portal will be sent to you via text once your request for consultation is approved.

Billing practice very deceiving, I should have been informed at the beginning of the call what the fee would be. Had I known, I would have cancelled immediately. The Dr was willing to reduce his fee, which I appreciate, but the appointment should never have proceeded as I did not sign the fee page, nor did I even know there was one. It came through this Luminaire website, which didn’t know about or was made aware of. Very deceiving in my opinion.
