Dr. Debra Utpadel, DNP, PMHNP-BC

Adult Psychiatric & Mental Health Nurse Practitioner
TX
At Zenses Health, I provide compassionate, evidence-based psychiatric care for adults through secure telehealth. My approach is collaborative and practical: I take time to listen, explain treatment options clearly, and help patients understand the “why” behind their care so they can participate confidently in treatment decisions. I provide adult psychiatric evaluations, medication management, and ongoing care for concerns including anxiety, depression, mood disorders, stress-related conditions, and life transitions. I also have a special clinical interest in adults exploring autism and neurodivergence, including sensory differences, masking, social exhaustion, and experiences that may have gone unrecognized earlier in life. With more than 25 years in nursing and over 13 years as a nurse practitioner, my background includes both primary care and psychiatric settings. This experience shapes a whole-person approach that considers the connection between mental health, physical health, lifestyle, and daily functioning. My goal is to create a comfortable, respectful environment where patients can ask questions, feel heard, and work toward realistic, sustainable progress.

What should a new client know about working with you?

Patients at Zenses Health can expect a thoughtful, collaborative, and nonjudgmental approach to psychiatric care. I understand that seeking mental health support can feel intimidating, particularly for adults who have spent years feeling misunderstood, dismissed, or uncertain about what they are experiencing. My goal is to create a comfortable environment where you feel heard, respected, and actively involved in your care. I don't believe in one-size-fits-all treatment. We'll consider your symptoms, medical history, lifestyle, goals, and preferences when making treatment decisions. I encourage questions and explain diagnoses, medications, potential benefits and risks, and other options so you understand not just what we're doing, but why. I work with adults experiencing anxiety, depression, mood symptoms, stress, burnout, and life transitions. I also have a special clinical interest in adults exploring autism and neurodivergence, including sensory differences, masking, social exhaustion, and experiences that may have gone unrecognized earlier in life. My style is conversational, practical, and education-focused. Medication is available when appropriate, but prescribing is not the entire visit. My goal is for you to leave appointments with a clearer understanding of your mental health and a treatment plan that makes sense for your life.

What is your typical process for working with clients?

The first appointment is a comprehensive psychiatric evaluation designed to understand you as a whole person, not simply a list of symptoms. We'll discuss your current concerns, mental health and medical history, previous treatment experiences, medications, sleep, stressors, lifestyle factors, and goals for care. Diagnostic clarification is an important part of this process, particularly when symptoms overlap or the picture has not previously been clear. I also have a special clinical interest in adults exploring autism and neurodivergence. When appropriate, we can discuss whether a more focused autism evaluation may be helpful and what next steps would make sense. At the end of the evaluation, we'll discuss my clinical impressions, treatment options, and recommendations together. I explain the reasoning behind recommendations and encourage questions so that treatment decisions are collaborative rather than something simply handed to you. Follow-up appointments focus on how you're doing, response to treatment, medication effectiveness and tolerability when applicable, and any new concerns. We'll adjust the treatment plan when needed and use education and practical strategies to support progress between visits. My overall approach emphasizes understanding what is happening, why it may be happening, and what we can realistically do about it. The goal is thoughtful psychiatric care built around communication, education, and a treatment plan that makes sense for you.
