“Derek is an amazing therapist. I originally came to him for meds management but he is absolutely going to be my therapist. I have always been resistant to male therapists but based on his reviews and qualifications, I decided to give it a shot. Derek actually listened and validated me. He took so much time to figure out my meds needs and I've never had a doc who would do that. Derek listened to everything I've been through and put me on plan that specifically addressed my issues, even taking in my post menopausal issues. I prefer not to treat symptoms, and rather get to the core of the issue. He's on board with that. If you're a woman on the fence about a male doc, don't blink an eye at him. He's a listener and someone who delves into the newest research for his patients. After 30 years, I think I found my therapist. ”

Hi! I'm Derek, a seasoned mental health professional with nearly 15 years of experience marked by a relentless pursuit of knowledge, diverse clinical experiences, and a commitment to enhancing mental health care across the lifespan. My work has specialized in psychotropic medications and psychotherapy, reflecting my dedication to providing integrated, comprehensive, holistic care. I am licensed in New York State in both Nursing and Social Work, holding credentials as a Nurse Practitioner (NP), Registered Nurse (RN), and both a master and clinical social worker (LMSW/LCSW). I also hold two ANCC board certifications in Psychiatric-Mental Health Across the Lifespan as both an RN (PMH-BC) and NP (PMHNP-BC). Over the last 15 years, I’ve worked outpatient as an NP providing medication management, inpatient psychiatry as an RN for Northwell Health and New York Presbyterian Hospital, and as a clinical social worker and psychotherapist. I’ve remained focused on complex, high-need mental health care for children and adolescents, while also dedicating myself to adults, geriatrics, LGBTQIA+, substance use, HIV/AIDS, clinical-forensic, intellectual/developmental disabilities (IDD), and serious mental illness (SMI). I earned my Master’s in Psychiatric and Mental Health Nursing from Stony Brook University and my Master’s in Social Work from Fordham University at Lincoln Center. I hold bachelor’s degrees from both Molloy University and Stony Brook University. As an ANCC Board Certified Psychiatric-Mental Health Nurse Practitioner, I specialize in psychopharmacological treatment for sub-specialty populations, including children/adolescents, LGBTQIA+, substance use/addiction (Opioid and Alcohol Use Disorder/Dependence), HIV/AIDS, and geriatric depression, anxiety, and dementia (cognitive dissonance, agitation, psychosis). My goal is to provide comprehensive, compassionate care tailored to each individual's unique needs.

What should a new client know about working with you?

At the time of booking intake paperwork will be sent automatically by Zocdoc to be filled out. There will be several questionnaires, some of which may not apply to your individual needs and you can just skip those specific ones. This will also include my contract. * The initial session is a 45 minute comprehensive psychiatric evaluation/assessment to discuss medical/mental health history, current mental health symptoms, and developing an appropriate treatment plan that will include either psychotropic medication management, psychotherapy/counseling, or both. * All subsequent follow up sessions are anywhere between 20-45 minutes depending on specific needs regarding medication refills or adjustments/changes, and psychotherapy/counseling. * *Please note that my primary focus is on the treatment of mental health with psychotropic medication management but I do incorporate a significant focus on psychotherapy/counseling and psycho-education into my practice* * And so, with all that being said, I thank you for taking the time out of your day to read and review my profile, and would be honored to work with you as the passenger and facilitator for healing on your mental health journey. What is your typical process for working with clients?

Initial Psychiatric Evaluation - I conduct my evaluations as a comprehensive and holistic dialogue between myself and clients that is more of a therapeutic conversation as opposed to a robotic check off list. This is a more person/client centered, humanistic and holistic approach that I consider as a much more effective way to develop a rapport between myself and clients. The therapeutic rapport/relationship is the backbone of treatment and is pivotal to the healing process! * See above for the aforementioned information regarding additional and subsequent sessions once our initial comprehensive psychiatric evaluation is completed. *

**Out of Pocket Rates** Initial Comprehensive Psychiatric Evaluation - $175 (45 minutes) Psych Medication Management & Rx Refill - $100 (20 minutes) Psychotherapy Only/No Rx Refill - $150 (45 minutes) I use Square payroll software to collect payments. This allows clients to receive a link with an invoice for clients to pay on their end without having to provide me with any of their payment information. You will automatically receive a receipt after payment as well.
