I'm Diane Guardiani, a board-certified psychiatric nurse practitioner (PMHNP-BC) with a Doctor of Nursing Practice and more than 25 years of experience helping adults find relief from depression, anxiety, ADHD, bipolar disorder, PTSD, and related conditions. What patients tell me they appreciate most is that I actually listen. I take the time to understand who you are, what you're navigating, and what you want your life to look like. From there, we build a treatment plan together — grounded in the best available evidence, tailored to your goals, and adjusted as we learn what works for you. What should a new client know about working with you?

I take a thoughtful, unhurried approach to medication. Medication can be a powerful tool, but it's not always the right first step — and even when it is, it works best alongside good sleep, meaningful connection, therapy, and the other pieces of a full life. I'll be honest with you about what medications can and can't do. I welcome questions. If a recommendation doesn't make sense to you, ask. If a medication isn't working, tell me. If you're skeptical about starting something new, let's talk about it. Good care depends on honest conversation, and I'd rather have the hard conversations than pretend they're not there. I believe in your autonomy. My job is to give you the best clinical guidance I can. Yours is to make decisions about your own care. You always have the final say — and you'll never be pushed toward a medication or plan you're not comfortable with. I coordinate care. If you're already seeing a therapist, I'll want to communicate with them (with your permission) so we're working toward the same goals. If you're not, I can help you find one — for most people, medication and therapy work best together. Between appointments, you can reach me through the patient portal for questions or concerns. I aim to respond within one business day, and for urgent situations we'll always have a plan in place.

What is your typical process for working with clients?

First appointment (60 minutes). We'll start with a comprehensive conversation about your history — current symptoms, prior treatment, medical background, family history, and what you're hoping to accomplish. If medication makes sense, we'll discuss the options together. If a different approach makes more sense, I'll tell you. You'll leave with a clear plan and understood next steps. Follow-up appointments (20 minutes). When starting or adjusting a medication, we typically meet every 2–4 weeks so we can monitor closely and make changes quickly if needed. Once you're stable, we'll shift to every 1–3 months. Each visit includes real time to talk — how you're actually feeling, what's changed, any side effects, and what to try next. Between appointments. You can message me through the patient portal for medication questions or non-urgent concerns. I respond within one business day. For urgent situations, we'll have a plan in place from your first visit. Ongoing collaboration. Treatment isn't static. We'll periodically reassess whether your current plan is still the right one — whether that means staying the course, adjusting doses, changing medications, or eventually tapering off. Your goals, not the medication list, drive the plan. Coordination with your care team. With your permission, I'll stay in touch with your therapist, primary care doctor, and any other providers involved in your care so we're all working toward the same outcomes.
