“Dr was over an hour late so all appointments were pushed back at least an hour. When I called to confirm my appointment (Since it was my first), I asked what to bring...I was told Insurance Card and Photo ID. Turns out that I needed MUCH more documents/documentation. Had I known, I would have brought LOADS of documents. I will have to fax a ton of paperwork tomorrow. ”
