Hi! I’m Edith Chak, a board-certified Psychiatric-Mental Health Nurse Practitioner with 10+ years of nursing experience and a strong background in interventional psychiatry. I earned my Master of Science in Psychiatric-Mental Health Nursing from Western University of Health Sciences after spending several years working in medical-surgical oncology and later specializing in advanced psychiatric treatments. My clinical experience includes comprehensive psychiatric evaluations, medication management, ketamine-assisted therapies, Spravato (esketamine), and Transcranial Magnetic Stimulation (TMS). I’m deeply committed to providing compassionate, evidence-based care that recognizes each person's unique experiences and goals.
What should a new client know about working with you?
I truly believe effective mental health care starts with listening. I choose to take time to understand each patient's concerns, strengths, and goals before developing an individualized treatment plan. My style is collaborative, trauma-informed, and person-centered, emphasizing shared decision-making. I strive to create a safe space where patients feel respected, supported, and empowered throughout their care.
Note: As a mental health nurse practitioner, I provide medication management, pharmacological support, and psychiatric care. I do not offer psychotherapy.
What is your typical process for working with clients?
I begin with a comprehensive psychiatric evaluation to better understand each person's symptoms, medical history, and treatment goals. Together, we develop a personalized care plan that may include medication management, lifestyle recommendations, psychotherapy referrals, and advanced biological treatments such as ketamine-assisted therapy, Spravato (esketamine), or Transcranial Magnetic Stimulation (TMS), if appropriate. I regularly monitor patients’ progress and adjust treatment as needs evolve, ensuring care remains individualized, evidence-based, and focused on helping patients achieve lasting improvement.

After scheduling your appointment, you will receive an email from Stella Mental Health (formerly BeSpoke Treatment) containing your intake paperwork. Please allow about 30 minutes to complete this form. Your responses will help your provider prepare for your upcoming appointment. All patients are required to complete the intake form 24 business hours before their appointment.
You will receive a phone call from a dedicated Care Advocate who can answer any questions you may have. Please be prepared to provide a credit card to keep on file for co-pays or cancellation fees. Late cancellations and no-shows will be charged the full initial appointment fee.
During your initial appointment, you’ll spend 60 minutes with your provider collaborating on a treatment plan tailored to your needs. If in-person treatments are recommended—such as TMS, Spravato, SGB, or ketamine therapy—our care team will guide you through the process and ensure you feel supported every step of the way.
