Dr. Fares Hosseinzadeh is a dedicated board-certified family medicine physician passionate about building lifelong relationships with his patients and their families. He believes in a personalized, whole-person approach to care focusing not just on treating illness, but on fostering wellness through every stage of life.
Dr. Hosseinzadeh completed his undergraduate studies at the University of Georgia and earned his medical degree from St. George’s University School of Medicine. He went on to complete his Family Medicine residency at Piedmont Columbus Regional, where he developed a strong foundation in comprehensive, evidence-based, community-focused care. Fluent in both English and Farsi, Dr. Hosseinzadeh takes pride in making patients from diverse backgrounds feel heard, respected, and empowered in their healthcare journey.

Very pleasant and has a good sense of humor. Like he has Saturday appointments which is a rarity these days. Has a lot of appointments open so he is more available then most, even though he's not right next door it's worth it to find a good doctor. Believe me I am picky. He refilled our prescriptions being our first visit. Next appointment in 3 months.

I needed an emergency prescription refill after moving and this location was the earliest available appointment that I could do with my work schedule. I was prepared to pay out of pocket, but they actually double checked my insurance and repeatedly worked with me to keep the costs as low as possible. Everyone was extremely kind and efficient, cannot recommend enough!

Dr. Hosseinzadeh and his Team made for an extremely efficient and comfortable annual check up/physical.
There are always some questions or concerns when establishing new primary care, but the feelings with Dr. Hosseinzadeh and his Team were nothing short of fantastic.
The office location being conveniently located off of State Bridge is a huge plus too.

Great doctors office. I had been sick for a few days and visited Family Health Primary Care. Dr. Hoss spent time in the room discussing treatment options I didn’t feel rushed at all. Even got to ask some wellness questions and he was very patient and knowledgeable. Answered every question for me. Highly recommend this primary care clinic in Johns Creek.

I had a really nice visit with Dr. Hoss. My concerns were addressed quickly and thoroughly, and he was able to refer me to a specialist he personally recommended, which I really appreciated.
The office was extremely clean, and I was seen within 15 minutes of checking in. In addition, parking was easy which was greatly appreciated after having a hard time with that in the city. I also had a follow-up question afterward, and Dr. Hoss personally called me to discuss it instead of routing me through a nurse or PA. That level of care and attentiveness truly stood out.

I had an appointment with Dr. Hoss the other day on short notice but was in dire need of establishing care with a primary care doc. I have some chronic medical conditions and Dr. Hoss was so attentive and caring with my issues. He was very thorough and professional, and the whole appointment was smooth and easy. I appreciate Dr. Hoss’s care, and I will definitely be back. Excited to have a dedicated primary care physician like him. Highly recommend.

Dr Hosseinzadeh was very patient and comprehensive with my initial visit. He took the time to understand my concerns and create a medical plan that works well for me and addresses my health goals. Him and his staff were very warm and welcoming and also efficient with vital signs and labs that were completed in the office. Very happy to have a primary care doctor that I trust in the community and will definitely be following up with Dr. Hosseinzadeh and his team for future visits.
