Farrah Jallalvandi

Adult Psychiatric & Mental Health Nurse Practitioner
CO
Hi, I'm Farrah Jallalvandi, a Psychiatric Mental Health Nurse Practitioner and founder of Flourish Psychiatry & Nursing. I see adults 18 and older in person in San Francisco and by telehealth across California, Colorado, Idaho, and Virginia. I don't believe in one-size-fits-all psychiatry. My approach is integrative and personalized, combining medication management, psychotherapy, and attention to your lifestyle, values, and goals. I look at the whole picture, including hormonal, neurological, emotional, and social factors that may be affecting how you feel. Education comes first in my visits. I'll explain what's happening in your body, why, and what your options are, so you can make informed decisions about your care. When medication is part of the plan, I prescribe and manage it with care and precision. Before becoming a PMHNP, I spent over 8 years as a registered nurse in psychiatric nursing, hospice and palliative care, home health, and clinical education. That background means I'm comfortable caring for patients with complex medical conditions alongside their mental health needs, including older adults and those needing memory care. I also mentor and train new psychiatric providers, which keeps my practice current and evidence-based. I believe the best outcomes happen when you feel informed, heard, and in control of your treatment. I offer new patient evaluations and ongoing follow-up care

What should a new client know about working with you?

Our first visit is a comprehensive evaluation where I'll get to know you, your history, and what you want to change. Expect to talk about more than symptoms: your health, sleep, relationships, stressors, and goals all help shape your plan. My care is collaborative. I'll explain what I think is going on and walk you through your options, and we'll decide on next steps together. You're never expected to just follow orders. A few practical things to know before you book: • Patient portal: After booking, you'll get a link to create a portal login. It's required for follow-up visits, so please set it up early. • AI scribe: I use an AI scribe for visit documentation so I can focus on you rather than my screen. Your consent is required to book. • Cancellations: Missed appointments or late cancellations carry a $100 fee for both new and follow-up visits. • Visit options: I see patients in person in San Francisco and by telehealth in CA, CO, ID, and VA. Come with your questions. Understanding your treatment is part of the treatment.

What is your typical process for working with clients?

Visit one: I ask you approximately 400 questions. Sleep, appetite, family history, how many tabs you have open right now (mentally and on your browser). It's thorough. It's a little nosy. It's my job. The Big Reveal: I'll explain what I think is going on in plain English. No medical jargon, no mysterious nodding, no dramatic "hmm" followed by silence. You'll learn what your brain and body are up to and why. Your brain is fascinating. Slightly dramatic, maybe, but fascinating. The Plan: Medication, therapy, lifestyle changes, or a combo platter. We build it around your actual life, not some imaginary version of you who meditates at 5 a.m. and meal-preps kale. Medication should fit your goals and lifestyle, not the other way around. You're the expert on you. I just have the prescription pad and an unreasonable amount of enthusiasm about neurotransmitters. Hormones, everybody has them: I grew up the only girl in my family, and now mom to two boys - so I speak fluent "men don't talk about their feelings" and can translate. I also know how much hormones shape mood for women at every stage, from PMS to postpartum to perimenopause and beyond, and how they shift for men over time, too. If something changed and nobody's connected it to your hormones yet, I will. The Tune-Ups: Finding the right medication is a bit like dating. Some look great on paper and are terrible in real life. Some are a little boring but treat you right. We keep going until we find the one (or the right combo). The goal: You, feeling like you again.
