You will begin care with a brief call with our New Patient Coordination Team. This step allows our clinical team to collect relevant medical history, confirm eligibility, and plan appropriate lab work in advance of your visit. Once complete, you will be scheduled directly with Dr. Seibold for your appointment at either his office in West Jordan, Utah, or Orem, Utah. Virtual Appointments are available for Utah residents.
