Graciana Ngah, APRN, NP

Adult Psychiatric & Mental Health Nurse Practitioner
992 Inwood Ave N, Oakdale, MN
I am a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) with over eight years of nursing experience and more than two years working in inpatient psychiatric settings. I provide comprehensive psychiatric evaluations, medication management, and supportive therapy for children ages 6 and older, adolescents, adults, and older adults. My clinical interests include treating a wide range of mental health conditions, including depression, anxiety disorders, bipolar disorder, ADHD, trauma-related disorders, autism spectrum disorder (ASD), substance use disorders, psychotic disorders, and other complex mental health concerns. My approach to care is grounded in Jean Watson’s Theory of Human Caring, which emphasizes compassion, authentic presence, and respect for the whole person—mind, body, and spirit. I believe that healing begins when individuals feel genuinely heard and understood. Through meaningful therapeutic relationships, empathetic listening, and shared decision-making, I work collaboratively with each patient to develop individualized treatment plans that align with their values, goals, and lived experiences. I am deeply committed to patient-centered and culturally responsive care. I strive to create a safe, nonjudgmental space where individuals feel respected, empowered, and actively involved in their mental health journey. I value open communication and believe that treatment decisions are most effective when made together, honoring each person’s voice and autonomy. I chose to become a Psychiatric Mental Health Nurse Practitioner because I am passionate about helping individuals achieve mental stability, improve overall functioning, and regain a sense of control and self-empowerment in their lives. My goal is to support lasting wellness by combining evidence-based medication management with supportive therapeutic interventions that promote resilience and long-term growth.
