Getting to know Hannah Fraley

I'm Hannah, a Board Certified Psychiatric Mental Health Nurse Practitioner serving patients in California, Nevada, and Hawai'i through Telehealth. I help children, teens, adults, and their families. As an Advanced Practice Registered Nurse with many years of experience in mental health, I work with individuals to better understand themselves and their needs, optimizing wellness. I am nationally board certified in Public Health and hold a Ph.D. in Population Health from the University of Massachusetts Boston. I earned a Master of Science in Nursing with a Psychiatric-Mental Health Nurse Practitioner specialty from Hawai‘i Pacific University, a Master of Science in Nursing Leadership from California State University, Fullerton, and a Bachelor of Science in Nursing from Simmons University. I am fully qualified to manage your psychiatric mental health care needs-I can evaluate, diagnose, and treat psychiatric conditions, prescribe medication, and offer brief supportive psychotherapy. I have extensive experience working with young people and adults facing common psychiatric conditions such as ADHD, Autism Spectrum Disorder, adjustment disorder, depression, anxiety, mood disorders, obsessions and compulsions, addiction, and sleep difficulties. I support parents and struggling teens, including those adjusting to an empty nest or other life challenges affecting mental health and well-being. I also help individuals heal from painful memories. My background includes serving as a Professor of Nursing for undergraduate and graduate students, and I have published research presented at several regional, national, and international professional conferences.

What should a new client know about working with you?

I work in collaboration with you or your loved one in developing a holistic treatment plan. This includes medication management and supportive psychotherapy as part of psychiatric medical visits allowing for longer, more personalized visits. I am a trauma-informed practitioner, and am able to provide you or your loved one comprehensive, integrative psychiatric and mental health services that is safe and supportive. My goal is to provide you or your loved one latest evidence-based psychiatric and mental health care that is whole-person centered.

What is your typical process for working with clients?

Initial Visits: We will meet for about 45-60 minutes via Telehealth online. Our goal is to build a trusting patient-practitioner relationship. We will discuss your main concerns, history and current symptoms - Our goal is to get to know you better. The comprehensive psychiatric evaluation will help us review what you are experiencing and begin a plan together for treatment. Follow-Up Appointments: After the initial visit, we will meet together to discuss how you have been since we last met. We will discuss your treatment progress and response to treatment and make any adjustments as needed. This may include discussing: Medications Lab Work Diagnostic Evaluations Holistic Interventions Supportive psychotherapy with every medical visit These visits are usually 15-30 minutes, sometimes 45-60 minutes if psychotherapy is part of your treatment plan. 15-30 minute sessions are more focused and are generally for clients who are meeting with other therapists, as we are addressing medications and holistic interventions only. Each individual treatment plan is tailored to the individual, but the frequency of visits generally range from weekly to every 1-3 months follow-up. We will develop your individualized follow-up plan together.

Hannah is BY FAR the most kind, caring, knowledgeable, and helpful psych I have ever had. I have felt so seen and understood from our very first meeting. She is knowledgable when it comes to working with queer people, audhd, dissociative systems and much more.
