Dr. Isaiah Crum, MD

Dr. Isaiah Crum, MD

Psychiatrist
NY
I am a board-certified psychiatrist treating adults across New York State. My practice is fully virtual. Most of my patients are doing well on paper and running on empty underneath. Anxiety that reads as being on top of everything. Depression that looks like being tired all the time. ADHD that got missed because your grades were good. Burnout that everyone around you keeps calling stress. I work most often with young professionals, and frequently with people carrying the particular weight of being the one who made it, the one who handles things, the one who is fine. That cost is real and we can talk about it plainly. What treatment looks like: your first visit is 50 minutes and covers your full history, not a symptom checklist. Follow-ups are 30 minutes and pair medication management with supportive therapy in the same session, so you are not repeating yourself to a stranger every month. I will explain what a medication does, what it costs you, and what happens if you stop. Nothing gets prescribed that you do not understand and agree to. I am not a replacement for ongoing psychotherapy. Many of my patients also see a therapist and I coordinate with them. If you would like one and do not have one, I will help you find a good fit. I see adults 18 and older. You do not have to be in crisis to make an appointment. You don't have to keep performing fine.

What should a new client know about working with you?

I'm direct. I'll tell you what I think is going on, why, and what I'd recommend, and I want you to push back when something doesn't fit. You're the expert on your life. I bring the medical side, and we make decisions together. You won't have to translate yourself here. Code-switching, being the only one in the room, carrying your family's expectations, the pressure to never look like you're struggling: we can name those things without a long explanation. Black patients and other patients of color are at the center of why I built this practice, and patients of every background, faith, gender, and sexuality are welcome. Practical things to know: - Visits are by video. Join from somewhere private, and you need to be physically located in New York State at the time of each visit. - Before your first visit, you'll complete an intake questionnaire. It's thorough on purpose, so our time goes to the conversation. - I'm not an emergency service. If you're in crisis between visits, call or text 988 or go to your nearest emergency room.

What is your typical process for working with clients?

Here's how it usually goes. Before your first visit: you'll complete an intake questionnaire covering your history, medications, and what's going on now. The more you share there, the more of our time goes to talking instead of catching up. First visit: a full psychiatric evaluation. By the end, you'll have my honest read on what's going on and a plan we agree on together. That plan may include medication, and it may not. If ADHD is part of the question, this is a clinical evaluation based on a detailed history and symptom assessment, not formal neuropsychological testing. The first few months: follow-ups every few weeks while we find what works. We track what's changing, adjust as needed, and keep room for what's happening in your life, not just your symptoms. Once you're stable: most people move to monthly visits or less often. The goal isn't to keep you in treatment forever. It's to get you feeling like yourself, with a plan you understand. Worth knowing up front: - New patients always start with a full evaluation. I don't provide one-time refills or single visits just for a prescription. - If you take a controlled medication like a stimulant, I follow New York's monitoring requirements and we'll keep a consistent follow-up schedule. - If you ever need more support than this model offers, I'll tell you directly and help you find the right level of care.
