My name is Ismel Fernandez-Calienes, an Autonomous Board-Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) with a strong commitment to compassionate, evidence-based mental health care. As the founder and lead clinician at ICare Mental Health and Wellness Services LLC, I provide psychiatric evaluations, medication management, and integrative treatment planning for children, adolescents, and adults experiencing a broad range of emotional, behavioral, and psychiatric conditions.
With a foundation in psychopharmacology and psychotherapy, I treat conditions such as depression, anxiety, ADHD, PTSD, bipolar disorder, autism spectrum disorder, and sleep disturbances. I believe in combining medication management with therapeutic interventions—including CBT, mindfulness, and psychoeducation—tailored to each individual’s unique needs and cultural background.
I take a holistic and trauma-informed approach, considering the physical, emotional, environmental, and spiritual aspects of health. My practice also integrates nutritional guidance, supplement education, and lifestyle changes to enhance clinical outcomes.
As an autonomous provider licensed in Florida and other states, I operate independently and take pride in offering accessible, judgment-free, and collaborative care. I also coordinate closely with primary care providers, therapists, and families to ensure a comprehensive support system.

What should a new client know about working with you?
At ICare Mental Health and Wellness Services, my approach is rooted in compassionate, evidence-based psychiatric care that integrates both medication management and therapeutic insight. As a Board-Certified Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC), I work collaboratively with patients to understand their unique psychological, emotional, and physiological needs.
My therapeutic style is:
• Empathic and Nonjudgmental – You’ll be met with respect, privacy, and unconditional support, no matter where you are in your journey.
• Structured and Goal-Oriented – We utilize CBT, motivational interviewing, and other integrative methods to help you develop insight, resilience, and practical coping skills.
• Culturally Attuned and Trauma-Informed – I honor your background and lived experience, and ensure that care is sensitive to past traumas, cultural values, and neurodiversity.
Medication Management Approach:
I believe in thoughtful, conservative prescribing with careful monitoring. We’ll regularly evaluate:
• Symptom improvement
• Medication adherence
• Side effects or concerns
• Functional progress at work, home, or school
I provide education so that you are fully informed about your treatment, including risks, benefits, and expected timelines. When indicated, I recommend routine lab work to monitor for medication safety and optimize your physical health (e.g., CBC, CMP, TSH, Vitamin D, Cortisol, Testosterone when appropriate).

Cancellation Policy:
