The patient intake process is designed for your convenience and comfort. It includes completing essential paperwork, reviewing insurance options, explaining our practice policies, and obtaining your signed consent forms.
The initial evaluation process involve conducting a comprehensive physical examination gathering a detailed medical and psychiatric history addressing your current symptoms and concerns while discussing potential diagnoses and treatment strategies. Together, we create a personalized care plan that actively involves you in decision-making, fostering open communication to build a trusting and collaborative relationship.
