“Jacob Read has—hands down—been the most available, compassionate, and effective mental health provider I've worked with. After a decade of evals, doctors, and intakes—I crossed "find good care" off my list and could move forward with "getting well." Therapeutically, as a prescriber, and professional, I believe Jacob cares about my life. For a patient like me who needs understanding, his sincere and consistent support over the last few years has made all the difference. (Thank you, Jacob, for all the work we've done.)”

“Absolutely by far the best therapist I have ever had and it has been a struggle. I had pretty much given up on therapy because all it ever consisted of was “how did it make it you feel?” I now think everyone should have a therapist, and the challenge is finding the right one. I am comfortable to talk with him about anything. It’s been a life- changer for me. ”

Getting to know Jacob Read

What excites me most about this work is helping people rediscover their strength—seeing that moment when healing and hope begin to feel possible again. My path to becoming a psychiatric-mental health nurse practitioner wasn’t a straight line. I left school early to work in construction, later earning my GED and eventually completing my master’s degree at Vanderbilt University. That experience taught me that growth doesn’t have to look perfect—it just has to be honest and persistent. It’s that same belief I bring to every patient I work with. I’m dual board-certified as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC) and Family Nurse Practitioner (FNP-C), and I specialize in treating depression, anxiety, PTSD, ADHD, and bipolar disorder. My approach is collaborative and grounded in compassion—I take time to listen, understand your story, and build a treatment plan that feels supportive and sustainable. I believe mental health care should never feel rushed or impersonal. Together, we’ll identify what matters most to you, set meaningful goals, and move at a pace that honors your comfort and growth. Whether we meet in person or through telehealth, my goal is to create a safe space where you feel heard, respected, and genuinely cared for.

What should a new client know about working with you?

When we begin working together, my first priority is to understand your story and what brings you to care. Our initial sessions focus on building trust, identifying your goals, and creating a treatment plan that feels both realistic and supportive. I take a thoughtful, evidence-based approach to medication management and often integrate Cognitive Behavioral Therapy (CBT) techniques into our work. We’ll move at a pace that feels comfortable while keeping your safety and well-being at the center of every decision. I’m careful and intentional when it comes to prescribing—especially with controlled substances—which are only considered after a thorough evaluation and ongoing collaboration. My goal is to help you find balance, stability, and renewed confidence through compassionate, personalized care.

What is your typical process for working with clients?

Our first session is about more than filling out forms or listing symptoms — it’s about beginning to truly understand you. During our initial hour together, we’ll take time to explore not only your medical and mental health history, but also the personal experiences that have shaped who you are today. We’ll discuss what’s worked, what hasn’t, and what “feeling better” realistically looks like for you. This session is collaborative from the start. I want you to feel heard, respected, and involved in every decision about your care. We’ll talk openly about treatment options, whether that includes therapy, medication, or a combination of both, and I’ll explain each recommendation so you understand the “why” behind it. My approach is structured but flexible—I’ll often help you set small, achievable goals so progress feels steady and visible. Between sessions, I may encourage journaling, mindfulness, or reflection to help connect insights from our conversations to daily life. Together, we’ll work to uncover patterns, build coping skills, and move toward a healthier, more grounded version of yourself. My goal isn’t just symptom relief—it’s lasting growth and confidence in your ability to manage life’s challenges.

Every visit I have with Jacob he asked me how I'm doing thus far into the week and feels like a genuine connection between therapist and patient. He really shows that he cares about his patients and their well-being.
