About Jennifer Weltsch
Jennifer is a board-certified Family Nurse Practitioner. She completed her Bachelor of Science in Nursing from Arizona State University in 2012. From there, she began her career as a Registered Nurse in an Intensive Care Unit. Determined to advance her education, Jennifer pursued and earned her Master of Science in Nursing from Grand Canyon University in 2024. She is a current member of the American Nurses Association and Sigma Theta Tau.
Jennifer is passionate about providing holistic, quality care that promotes health and well-being. She believes her patients should be active participants in managing their healthcare, as she feels shared decision-making is vital to positive patient outcomes. Jennifer also emphasizes the importance of offering treatment options based on the best evidence-based recommendations. Her goal is to establish patient relationships that foster open communication and improve long-term outcomes for all her patients.

I am very excited to shift my career focus from managing those in critical condition to promoting health and preventing disease progression. My years working in critical care gave me the opportunity to advocate for countless patients, and I have learned how important it is for all patients to feel heard. I value patients' beliefs and want to ensure that everyone under my care receives the very best treatments.
I am an Arizona native and have lived throughout the state. When I am not working, I enjoy spending time with my husband, family, and friends. For fun, I enjoy reading, traveling, listening to podcasts, and playing video games with my husband.

I booked my appointment via Zocdoc app, user friendly platform with various open time slots to accommodate my health concern. Clear instructions provided to prep for my in-office visit. MY DR NOW was great all-in-all! Very Courteous staff via phone and in person?. Clean facility and good vibes with Dr Dre bumping from speakers:) I’d recommend both zocdoc and MY DR NOW. Great doctor visit.

I am so happy I found Jennifer Grim NP. I had not had a PCP and was searching. Her amazing reviews caught my attention. she and her office were professional in every way. So friendly, check in was easy, Wait time was minimal to none. Jennifer was so incredibly nice and caring and really listens to you. You certainly don’t feel rushed and you feel heard. She took the time to listen to my concerns and established a plan. I would 100% recommend her and this office.

My first visit with Jennifer was amazing! She was extremely helpful, kind, and patient. She listened to my concerns and gave me several different options that she recommended I research and follow up on during a second appointment. It felt very professional and courteous. What a great experience!

Wait times are always minimum, staff always friendly and professional. Jennifer Grim is an excellent medical provider in all aspects. You don't feel rushed and detailed explanations are always given. Canyon Family Medicine makes you feel like they care. You are confident the best care and medical advise/procedures are offered.

So glad I happened to find this clinic for my care while in Arizona. Jennifer Grim FNP is my primary care doctor while I’m here and I have so much confidence in her, she is just great! This was my first visit at Canyon Family Medicine. Reception was kind and respectful. The Medical Assistant was competent and sweet. Jennifer was a ray of sunshine for me. She seems very knowledgeable and made me feel welcomed and relaxed. Her demeanor is pleasant and non-rushed. Great to have an interaction with a provider who actually makes eye contact and listens! I think I found a new medical home!
