Getting to know Jilian Hammond

I'm Jilian Hammond, a board-certified psychiatric-mental health nurse practitioner and founder of Hammond Health. I provide compassionate, personalized care to support you on your path to healing, balance, and a more fulfilling life. Whether you're seeking help for anxiety, depression, ADHD, or other mental health concerns, you're not alone—I'm here to walk alongside you every step of the way. My journey into mental health care began as a certified nursing assistant, where I first discovered my passion for helping people feel seen, heard, and genuinely cared for. That same dedication continues to guide my practice today. I earned my PMHNP degree from Maryville University while working as a nurse at OSF Saint Francis Medical Center. Based in the Peoria, IL area, I offer virtual care to adults ages 18–65 throughout the state of Illinois. My appointments include thoughtful medication management, with supportive, therapy-informed conversation woven into each visit.

What should a new client know about working with you?

My approach blends thoughtful medication management with supportive, therapy-informed conversation—so every visit focuses on your whole experience, not just symptoms. Together, we’ll explore what’s going on for you, develop personalized strategies, and adjust treatment as needed to help you feel better

What is your typical process for working with clients?

Your first appointment is a comprehensive 60-minute session where we’ll explore your mental health history, current concerns, and treatment goals. This is a space for us to get to know each other and determine the best plan forward for your care. After your first appointment we will have ongoing follow-ups are 30-minute check-ins to monitor your progress, adjust medications as needed, and provide continued support. These sessions are tailored to help you stay on track and feel empowered in your mental health journey.
