When I reached out for a form, I wasn’t immediately directed to this authorization. I should not have to provide any info to get a blank medical release form for the office. I was forced to go to their website and search for the form myself…. unbeknownst to Udo. ??”

At our first appointment, she said she would send me questionnaires to fill out so that she can diagnose me at our second appointment. It's 24 hours later, and she hasn't sent me anything. If it's urgent, don't choose this provider.

I did a lot of research and picked a doctor whom I couldn’t wait to see. At the last minute, the office changed my appointment to someone I didn’t want. At that point it was too late to cancel the appointment without being charged a $50 cancellation fee. They claim the doctor I picked doesn’t take my insurance even though she was listed as someone who did on Zoc doc. They shouldn’t list a doctor as in network when they aren’t. It was a weird bait and switch to get clients, and I didn’t like it at all.
The NP who saw me also made a lot of assumptions about me based on her own personal experience. Just because I am Nigerian like you doesn’t mean I have the same experience as you do.
