About Kaley O'Neill
We are pleased to share exciting news about our care team at Commonwealth Medical Practice Chelmsford. Beginning this month, Kaley O’Neill, MSN, FNP-C, will be joining our clinic as a Family Nurse Practitioner.
Kaley is a compassionate and motivated provider with a strong background in adult primary care. She is experienced in managing both acute and chronic conditions, performing thorough physical exams, and developing personalized treatment plans. Kaley also brings a passion for patient education, preventive care, and collaborative team-based medicine.
Kaley will be available Monday through Friday, 9:00 AM to 4:00 PM, working alongside Dr. Sebastian Sepulveda to ensure that our patients receive high-quality, coordinated care.
We are confident that you will find Kaley’s knowledge, warmth, and dedication to patient well-being to be a valuable addition to your care experience here at Commonwealth Medical Practice.

Staff was very friendly without seeming condescending, got me in very fast, and Kaley's a great listener as well as helpful! Not always easy to balance kindness with professionalism, but these guys make it seem that way.

I’ve spent the last few months dealing with the chronic illness and very little to know medical prioritization or help through major hospital chains.
