Getting to know Kathryn Montierth

If you have tried several psychiatric medications and the plan still does not make sense, I can help you sort through it. I’m Kathryn Montierth, MSN, APRN, PMHNP-BC, a board-certified psychiatric mental health nurse practitioner providing telehealth care across Arizona. I work with adults and teens experiencing ADHD, anxiety, depression, bipolar disorder, OCD, trauma-related symptoms, panic, insomnia, and other mood and attention concerns. I also work with people who have had side effects, only partial improvement, or lingering questions about whether the diagnosis and treatment plan still fit. My approach starts with understanding the whole picture. I review what you've tried, what helped, what didn't, medication effects, sleep, medical history, and how symptoms have changed over time. I explain my clinical reasoning in plain language so you understand why I am recommending something, not just what I am prescribing. From there, we build a plan together and adjust it thoughtfully over time. I bring 14 years of nursing experience across psychiatric crisis care, inpatient psychiatry, neurology, and complex medical care. That background helps me look beyond a single symptom or diagnosis when psychiatric and medical factors overlap. I provide psychiatric evaluations, medication management, diagnostic clarification, second opinions, and ongoing follow-up. ADHD treatment is individualized after a comprehensive assessment, and medication decisions are based on clinical appropriateness rather than a predetermined prescription. Yarrow Behavioral Health is a solo practice, meaning you will work directly with me during each visit. I offer appointments on the following days and times: - Tuesdays, Saturdays, and Sundays: 5:00 AM to 7:00 PM I provide early morning, daytime, evening, and weekend appointments for patients who require care outside the traditional workweek.

What should a new client know about working with you?

My role is focused on psychiatric evaluation and medication management. I do not provide ongoing talk therapy, and many of my patients already work with a therapist. With your permission, I am happy to coordinate with your therapist or other treating clinicians so each part of your care is working toward the same goal. Before your appointment, it is helpful to have an accurate list of your current medications and, when possible, information about medications you have taken in the past. Please be open about side effects, missed doses, supplements, alcohol or substance use, and relevant medical issues. Those details matter for safe prescribing and good decision-making. Visits are conducted via secure video. Please be physically in Arizona during the appointment and join from a private place where you can speak openly. If you plan to use insurance, complete any requested insurance verification and provide payment information before the appointment to avoid unnecessary delays. My practice is outpatient and is not designed for psychiatric emergencies or crisis stabilization. If you are experiencing an emergency, call or text 988, call 911, or go to the nearest emergency department.

What is your typical process for working with clients?

Before your first visit, you will complete intake information and any screening measures appropriate to your concerns. Prior psychiatric records, medication lists, or relevant laboratory results can also be useful when available. The first appointment is a comprehensive psychiatric evaluation. I assess your current symptoms, psychiatric and medical history, previous treatment, sleep, functioning, family history, and other factors that may affect the diagnosis or medication plan. When symptoms overlap, I may use additional screening tools or recommend obtaining records or laboratory testing before making certain decisions. At the end of the evaluation, I will explain my diagnostic impression, what I am still considering, and the treatment options that make sense. A prescription is not automatic simply because an appointment was booked. Sometimes the best next step is medication; other times it is gathering more information before changing treatment. Follow-up visits assess response, side effects, functioning, and whether the treatment plan needs to change. Follow-ups may be closer together while medications are being adjusted and become less frequent once the plan is stable. When possible, I prefer deliberate medication changes so we can clearly tell what is helping and what is not. If you also work with a therapist, I can coordinate care with your permission while remaining focused on the psychiatric and medication-management side of treatment.
