Im Dr. Kimberly Clayton DNP MBA PMHNP-BC AGPCNP-C APRN im Dual board certified by American Nurses Credentialing Center (ANCC) as a Psychiatric Mental Health Nurse Practitioner and certified by The American Academy Of Nurse Practitioners as an Adult Gerontology Primary Care Nurse Practitioner. Dr. Clayton has over 29 years' experience in medical field between her hometown of Meridian, Ms and currently in Jacksonville, Fl. As a dual-certified nurse practitioner, I understand the complexity of mental health, wellness, health promotion, and the effects of stress and chronic illness. Dr. Clayton is licensed in Florida, Georgia, South Carolina, North Carolina, Virginia, Tennessee, and Washington State . I provide psychiatric evaluations and medication management with individualized care for every patient. I treat a variety mental health issues such as: Depression, ADHD, Schizophrenia, Anxiety, Mood Disorders, PTSD, conduct disorders, Insomnia, Dissociative disorders, Personality disorders, Addiction and other psychiatric disorders. She also offers one-on-one psychotherapy. Your mental health is MY priority. I am committed to offering the best support and treatment for mental illnesses to improve your quality of life. Our vision is to create awareness on mental health and ensure IM easily accessible to all who need mental health care. Payment is due after each session and will be charged with your card on file for any portion of patient responsibility. Receipts will be provided at the time of the charge. This typically takes place 2-4 weeks after the session, once your insurance has had a chance to process the claim. We try our best to verify eligibility, but it is ultimately your responsibility to check coverage and out-of-pocket costs for services. For out-of-network visits, you will be billed at the provider’s self-pay rate, and we can provide an invoice for you to submit to your insurance for potential out of-network reimbursement based on your plan’s benefits.
