“Dr Peralta is SUPER nice and friendly. My experience was very quick in and out for a routine physical and she had no problem addressing things I was concerned about, ie my blood pressure. Told me about options and her opinions. Will definitely be making her my new doctor. Waiting room is smaller than you’d see in a huge practice, but it’s cozy and gets the job done. Very clean. Super nice staff. 10/10 would recommend. ??”

We collect deductibles, copays at the time of service. If you have any questions or concerns, please call your insurance before your visit. In addition, please be accurate about the reason for your visit. If you cannot find the reason for visit listed on my Zocdoc profile, that means I do not see patients for those reasons. You will be responsible for any charges associated with reserving the appointment if I need to cancel your appointment. Leslyn Peralta is a dedicated, board-certified Family Nurse Practitioner (FNP) by the American Academy of Nurse Practitioners. Prior to becoming a nurse practitioner, Leslyn gained years of experience as an inpatient registered nurse. She earned her Associate's degree from San Diego City College, a Bachelor's degree from the University of Louisiana, and a Master of Science in Nursing from Walden University. Her goal is to educate patients on how to achieve a healthy lifestyle.

The provider I saw was NP Peralta. This was my first time meeting her to establish care. She was wonderful in listening to my concerns and getting me the referrals and tests that I need. My follow up appt was scheduled before I left.

Dr. Peralta listens to everything I have to say and covers all the bases in helping me get better. She is great with referrals and goes down the list of possibilities to ensure we get to the issue. Highly recommend.
