Please review a few things when booking with me: Before booking, please make sure the location works for you. We have a number of locations in the Inland Empire. We collect deductibles, copays at the time of service. If you have any questions or concerns, please call your insurance before your visit. In addition, please be accurate about the reason for your visit. If you cannot find the reason for visit listed on my Zocdoc profile, that means I do not see patients for those reasons. You will be responsible for any charges associated with reserving the appointment if I need to cancel your appointment. If you have an insurance we do not accept, you will be charged cash for those appointments. We do not prescribe controlled substances. Leslyn Peralta is a dedicated, board-certified Family Nurse Practitioner (FNP) by the American Academy of Nurse Practitioners. Prior to becoming a nurse practitioner, Leslyn gained years of experience as an inpatient registered nurse. She earned her Associate's degree from San Diego City College, a Bachelor's degree from the University of Louisiana, and a Master of Science in Nursing from Walden University. Her goal is to educate patients on how to achieve a healthy lifestyle.
