Adult Psychiatric & Mental Health Nurse Practitioner
4 Ct St, Ste 204, Plymouth, MA
I am Michael MacDonald, MSN, APRN, a dual board-certified Nurse Practitioner in Psychiatric-Mental Health (PMHNP-BC) and Adult-Gerontology Primary Care (AGPCNP-BC). I have more than eight years of experience caring for adults in Massachusetts, including complex medical and psychiatric needs, addiction and detox medicine, and long-term medication management. My practice, Silver Crown Mental Health in Plymouth, is built around a simple idea: you should be known, not processed. I keep a small panel so appointments can be unhurried and care can stay personal. I look at the whole picture—mood, sleep, focus, stress, substances, physical health, relationships, work, and the daily habits that either support recovery or quietly undo it. People come to me for depression, anxiety, ADHD, bipolar spectrum conditions, trauma-related symptoms, insomnia, and co-occurring medical issues. I also work with high-performing adults who look fine on paper and still do not feel well. My background as an exercise physiologist and primary care NP helps me think beyond the prescription pad when labs, blood pressure, hormones, sleep, or lifestyle are part of the problem. I completed my psychiatric mental health postgraduate certificate at Wilkes University and hold national board certification through ANCC. Visits are longer than a typical medication check, with time for diagnosis, education, and a clear plan. Care is private-pay. I do not bill insurance directly, and I can provide a superbill if you want to seek out-of-network reimbursement through HSA, FSA, or your plan. I see patients in person in Plymouth and by telehealth when appropriate. With your permission, I coordinate directly with your therapist and primary care provider so everyone involved in your care is working from the same plan. If you want careful diagnosis, thoughtful medication management, and a clinician with time to think with you, I welcome new patients and invite you to book a visit.

What should a new client know about working with you?

Working with me is a concierge psychiatry membership, not a typical insurance visit. Silver Crown Mental Health is a small, private-pay practice in Plymouth. Membership is per year. That fee covers your care for the year: longer appointments, careful diagnosis, medication management, a personalized plan, and direct access to me between visits for clinical questions, refills, and adjustments. I keep a limited panel on purpose. You are not one of hundreds of charts. Visits are unhurried. We look at the whole picture—mood, sleep, focus, stress, substances, physical health, work, relationships, and the daily habits that either support recovery or quietly undo it. I am dual board-certified in psychiatric mental health and adult-gerontology primary care, so medical issues are part of the conversation, not an afterthought. New patients start with a comprehensive evaluation. From there we build a clear plan and follow it together. I prescribe when it is clinically appropriate and send prescriptions electronically to the pharmacy you choose. With your permission, I coordinate with your therapist and primary care provider so treatment stays aligned. You should expect honest recommendations, a measured approach to controlled medications, and follow-up that matches how you are actually doing, not a rushed 15-minute refill. I do not bill insurance directly. If you want to seek out-of-network reimbursement, I can provide a superbill. HSA and FSA funds may be used. Care is available in person and by telehealth when appropriate. This is not a high-volume clinic and it is not a drop-in medication mill. It is for people who want consistent follow-through and a clinician who has time. If that is what you are looking for, the annual membership is $10,000. A brief consultation can be scheduled first; if you join, that consultation fee is credited toward the membership so you are not paying extra to get started.
