I was amazed at the questions asked on the form when I got there.
Very long form, lots of questions that were totally unnecessary.
I kept thinking what does any of this have to do with cleaning wax out of my ears?
This would be appropriate for a mental facility, a psychiatric facility.
Not what I was there for.
I went to a new dentist, they simply asked what physical illnesses I have had.
Definitely not a mental illness intake.
Again, definitely NOT what I was there for.
If I was I would have sought out such an office.

The office was very welcoming, the office was very fast with labs and very friendly staff. Clean rooms and quick service, the ma was very friendly and the doctor listened to me and did not rush thru my appointment, I will definitely keep this as my main doctors office for all my future needs.
