What is your typical process for working with clients?

We look forward to seeing you at your upcoming appointment. To keep your visit as smooth as possible, please complete your new patient intake forms in advance. After scheduling your appointment time through Zocdoc, you’ll receive an email from Headway with a link to create your patient profile and fill out all required forms in the Headway portal before your appointment. We reserve this appointment time just for you. Because last-minute cancellations and missed visits make it harder for other patients to access care, we ask that you let us know at least 24 hours in advance if you need to cancel or re-schedule or this will result in a $75 late cancellation fee.
