I now see clients in my own practice, providing individual patient centered care for adults. I have a concierge-style model, focusing on personalized and collaborative care. What should a new client know about working with you?
Thank you for scheduling through Zocdoc. Visits are conducted by telehealth video. You will recieve a link to join; please use a device with working microphone and video.
Forms will be sent to complete when the appointment is scheduled.
Please complete promptly so your information can be verified before your visit.
Once everything is complete and verified, we will ready for your appointment.
Headway will send you forms through a patient portal immediately upon scheduling your appointment. Once they are completed and send back I am cleared to start the visit.
What is your typical process for working with clients?
The initial assessment will include a 45 minute detailed psychiatric evaluation with medical history included.
You can expect a warm, confidential space focused on your concerns and tailored to your needs.
