Hi, I’m Rachel Pierre-Chery, FNP-C, a certified Family Nurse Practitioner with more than six years of experience providing compassionate, patient-centered care. I believe healthcare should be personal, respectful, and easy to understand. My goal is to create a comfortable environment where patients feel heard, supported, and actively involved in their health decisions.
Throughout my career, I have cared for patients with a wide range of acute and chronic health concerns in primary care and community health settings. At Here For You Health Care, LLC, I provide convenient care through telehealth and mobile visits, with a focus on meeting patients where they are and making healthcare more accessible.
My services include routine and follow-up care, medication management and refills when appropriate, lab review, preventive care, chronic disease management for conditions such as diabetes and high blood pressure, weight management, and evaluation of common illnesses and health concerns. I also incorporate screening for anxiety and depression because I believe emotional and physical health are closely connected.
I take time to listen to my patients, explain diagnoses and treatment options in clear language, and develop care plans that are realistic for each person's lifestyle and needs. I want my patients to feel comfortable asking questions and confident about the next steps in their care.
I earned my Master of Science in Nursing as a Family Nurse Practitioner from Grand Canyon University and my Bachelor of Science in Nursing from the University of Phoenix. I am also continuing my education in psychiatric-mental health care to further strengthen my ability to provide whole-person care.
