Hello! I’m pleased to introduce myself. I am a Psychiatric Mental Health Nurse Practitioner (PMHNP) with extensive experience in long-term care settings as well as medical-surgical (med-surg) environments. My passion lies in providing comprehensive mental health care to individuals across various stages of life, with a particular focus on integrating physical and mental health.

After You Book an Appointment
After scheduling, you’ll receive an email from your provider via SimplePractice, our secure client portal. This email includes all required intake forms.
Forms Required Before Your Appointment
The following must be completed in the SimplePractice portal at least 24 hours in advance:
Intake and consent forms
Credit card information on file
If you book within 24 hours of your appointment, all forms must be completed within 2 hours of receiving them, and you must be in contact with the provider.
Medication Policy – Especially Regarding Stimulants
Stimulant medications (commonly used for ADHD) are not prescribed at the initial appointment. They are considered on a case-by-case basis after a full evaluation and at the provider’s discretion.

Your provider will also send a message to confirm your appointment and answer any questions.
If you do not respond, your appointment may be canceled. By not responding, you are agreeing to the cancellation. Please contact our office if you have any concerns.

If any forms are incomplete or payment information is missing, your appointment will be automatically canceled.
