Important Information Before Your Appointment: Intake Forms Once your appointment is scheduled, please complete the intake forms before your visit. These forms help us understand your medical, psychiatric, and family history, allowing us to provide the best individualized care. Payment and Insurance We accept both private pay and insurance. A valid credit card is required to reserve your appointment. An $80 fee will be charged for no-shows or late cancellations (less than 24 hours ' notice). You are responsible for any unmet deductibles or co-pays. Please verify your benefits directly with your insurance provider. Forms and Documentation Please note that we do not complete paperwork related to: FMLA (Family and Medical Leave Act), LOA (Leave of Absence), Disability claims, or Workers’ compensation. ADHD Evaluation and Treatment Stimulant medications (such as Adderall, Vyvanse, or Concerta) may be prescribed only after a comprehensive evaluation. If you already have a documented ADHD diagnosis and are taking stimulants, please bring your previous medical records to your first visit, or sign a release of information so we can obtain them from your prior provider. Benzodiazepine Policy Benzodiazepines (such as Xanax, Klonopin, Ativan) are not prescribed for daily or long-term use. If you are currently taking these medications, we will discuss a safe and gradual tapering plan as part of your treatment process. Controlled Substances and Follow-Up If a controlled substance is prescribed, regular follow-up appointments will be required to ensure safety and effectiveness. Periodic urine drug screenings may be requested to ensure your safety.

Diagnosis: Based on the assessment and any additional evaluations, the specialist makes a diagnosis that guides treatment and helps the client understand their condition. 3. Treatment Planning: The specialist collaborates with the client to create a comprehensive treatment plan, which may include medication, psychotherapy, lifestyle changes, and crisis management. 4.
