Hi, I'm Dr. Rebecca Osborne Lewis, the founder of Generational Care, LLC. I am a board-certified family medicine physician. Since I completed residency, I have seen a horrible trend in medicine where the patient doesn't seem to come first anymore. Instead of working to keep our patients healthy, our healthcare system seems to be more focused on treating medical conditions and earning even more money. After an unexpected family tragedy in 2022, I gained the courage to make a change - leaving a large healthcare system and starting my own practice. Office Policies - AS PER STATE LAW, PATIENTS MUST BE IN KENTUCKY AT TIME OF VISIT. - ALL VISITS ARE COMPLETED VIA TELEHEALTH. - Payment for visit is expected WHEN PAYMENT LINK IS RECEIVED. It is sent via text to phone number provided and email provided as long as email isn't apple/icloud. - AS PER FEDERAL LAW, if you have MEDICARE of any type, KY medicaid, or Tricare, it MUST be used. - Medicare patients are expected to pay co-pay prior to visit and visit will be submitted to insurance. Currently, I am only in-network for Original Medicare. If out-of-network, you must pay full cost of visit and then will be reimbursed by Medicare after billed. I am unable to see TRICARE or medicaid patients at this time. - Patients paying at the time of service without using insurance, a new patient 30 minute video visit is discounted to $79. Additional time will be charged at $20 per each additional 30 minutes. - NO SHOW FEE. There is a $30 no show fee if appointment is missed or not cancelled prior to 24 hours of the appointment time. The remainder of the $79 will be refunded if this happens. If the appointment is cancelled prior to 24 hours, then $79 will be refunded. Failure to complete payment in a timely manner could result in appointment being cancelled. -Make sure to complete ALL intake forms as soon as possible so they can be reviewed prior to appointment.
