Getting to know Riley Guinan

My name is Riley, and I am a board-certified physician assistant practicing at Zellig Psychiatry. We are a small, thoughtfully run practice dedicated to evidence-based psychiatric evaluations and medication management. I work with teens and adults navigating a wide variety of emotional and cognitive challenges, with particular expertise in ADHD, depression, and anxiety disorders. I earned my undergraduate degree in psychology from Rutgers–New Brunswick and my Master’s in Physician Assistant Studies from Thomas Jefferson University. My background includes academic research in psychiatry as well as advanced training in psychopharmacology through the Neuroscience Education Institute (NEI). I’ve worked in both inpatient and outpatient settings with pediatric and adult populations. I greatly enjoy getting to know each of my patients and working collaboratively to craft individualized treatment plans. This means considering wide-ranging factors such as genetics, lifestyle, sleep, nutrition and more. Our path forward may include medication when appropriate, targeted supplements, behavioral strategies, or a combination of these approaches. We will leave no stone unturned in figuring out the best possible plan for you. I look forward to welcoming you to the practice and helping you feel your best.

What should a new client know about working with you?

I bring an analytical lens to every case, viewing each patient’s journey as a unique puzzle. I truly enjoy going the extra mile, whether that means diving deep into complex clinical research or collaborating closely with my partner at the practice, a Johns Hopkins-trained psychiatrist, to ensure you receive the highest level of care. I also pride myself on being exceptionally responsive. I understand that mental health challenges don't always follow a 9-to-5 schedule, so I prioritize communication to a degree that is rare in the healthcare field. When you work with me, you are a partner in a thoughtful, evidence-based process where we prioritize getting each detail right.

What is your typical process for working with clients?

Initial appointments are 60 minutes. During this visit, we take a comprehensive look at what has been going on recently, your psychiatric and medical history, prior treatments, what's worked, and what hasn't. The goal is to develop a clear and accurate understanding before making decisions. In many cases, we schedule a follow-up about one week later for 30 minutes. This allows us to continue clarifying patterns and and refine our understanding of your concerns. Once we have a solid clinical picture, we develop a plan together. This may include medication when appropriate, behavioral strategies, lifestyle adjustments, or further evaluation. Ongoing follow-ups are structured and goal-oriented, with regular reassessment to ensure we are moving in the right direction.
