My goal is to help individuals & families learn how to manage and overcome life challenges irrespective of their mental health diagnosis. I am a good listener, and using current research, clinical judgment, along with patient preference, we can treat/manage many mental health diagnoses

What is your typical process for working with clients?

At Holsoms TMS, we have created an easy workflow for the convenienc of our patients and providers. You will expect: ADHD DIAGNOSTIC ASSESSMENT REQUIRES AN ADDITIONAL SESSION OF COMPLETING THE TESTS REQUIRED BEFORE MEETING WITH THE PROVIDER TO DISCUSS THE OUTCOME-TYPICALLY AROUND 30 MINUTES. 1. Intake Forms Completion (Pre-Appointment or Upon Arrival) Patient responsibilities: Complete electronic or paper intake forms, including: Demographic information Presenting concerns Medical & psychiatric history Substance use history Social & developmental history Family history of mental illness Consent forms (treatment, telehealth, HIPAA) Staff responsibilities: Review completed forms for completeness and flag urgent concerns. Ensure patient understands confidentiality and rights. 2. Diagnostic Evaluation Session (60–90 Minutes) Conducted by PMHNP Establish rapport through open-ended dialogue; use psychoanalytically informed listening to explore the patient's narrative. Explore presenting problem(s): Onset, duration, severity, impact on functioning. Review psychiatric history: Past diagnoses, hospitalizations, previous therapy, or psychiatric care. Symptom inventory: Current and past symptoms (mood, anxiety, psychosis, trauma, neurovegetative, etc.) Mental status exam (MSE): Observe appearance, behavior, speech, affect, thought processes, cognition, insight, etc. Functional assessment: Social, academic, occupational functioning. 3. Medication Reconciliation Review all current medications (psychiatric & medical). Evaluate past psychotropic medications: efficacy, side effects, adherence. Document allergies and adverse drug reactions. Coordinate with PCP or pharmacy if needed. 4. Formulation & Diagnosis Develop a working diagnosis (or differential diagnoses) using DSM-5-TR criteria. Integrate biological, psychological, and social contributors. A psychoanalytic lens is used to understand deeper relational patterns or defenses, as applicable. 5. Collaborative Treatment Planning
