Getting to know Shannon Dingle

I have always worked in the mental health field. My early years involved coordinating services for people living with severe and chronic psychiatric illness. I also spent time working in research; these were important years because they taught me how to truly listen to an individual relate their story. There is a statistic that physicians interrupt patients within the first 30 seconds of an encounter. I keep that in mind, allowing the patient to express themselves freely. I hold a Master’s degree in Psychiatric Mental Health Nursing from Columbia University, as well as bachelor’s degrees from Mercy College and the University of Rochester. As a psychiatric NP, I am trained in the pharmacologic management of psychiatric illness. I am also trained in therapy and try to address any psychosocial stressors the patient is experiencing, in addition to monitoring responses to pharmacotherapy. At the initial session, I conduct a thorough evaluation that helps determine if I will recommend medication. However, each person needs to come to that place on their own. I don’t expect every initial session to end with a prescription. Some individuals need time to process the encounter and decide if they want to move forward with medication. And that’s just fine.

What should a new client know about working with you?

I have a very relaxed style. I try to be relatable; yes, occasionally I make jokes. I recognize even considering psychiatric medicine is hard for most. I spend a lot of time providing education on the medications. Sometimes I send patients to other sources so they can read up. It's a collaborative process. I don't make the decisions. We decide, together, if and when we should start a medication/change dosage, etc.

What is your typical process for working with clients?

The initial session will be a thorough psychiatric evaluation, where I learn about the patient's social, developmental, medical and psychiatric histories; the last one will include treatment episodes and previous psychiatric meds, if any. At the end of the session, I will tell the patient my assessment, as well as my recommendations. I ask the patient their own goals for treatment and document. Sometimes I'm continuing their current meds; sometimes I make a new recommendation. The patient may need time to decide, so we discuss at the next session which is usually within 4 weeks. Subsequent sessions will be shorter; we will monitor efficacy or lack thereof of the medication regimen. I also try to stay abreast of current psycho-social stressors the patient may be facing. Sometimes folks have unrealistic expectations of the medication; they may actually need to work on some issues in therapy. I strongly encourage all patients to be engaged in therapy concurrently; studies show the best outcomes when patients take medication, in conjunction with talk therapy.
