My name is Dr. Siyami Takha. I treat patients with a variety of mental health conditions, including depression, anxiety, bipolar disorder, PTSD, schizophrenia, substance use disorder, and borderline personality disorder. I have the privilege of working with a diverse population, providing culturally competent care and helping to eliminate stigmas associated with mental health. I would be honored to help you navigate your mental health journey. Together, we will empower you with coping skills to lead a productive life. Life's changes-whether anticipated or unexpected-can be challenging, and sometimes we need new tools to manage these transitions. I assess, diagnose, and treat mental health conditions with this in mind. I apply a holistic approach to problem-solving, looking at the whole person-not just their mental health needs. I consider physical, emotional, social, and spiritual health to determine the best course of treatment for each patient. My goal is to provide comprehensive care that addresses all aspects of well-being. I focus on supportive and talk therapy and am passionate about equipping clients and families with tools to understand and manage behaviors, leading to more balanced and meaningful lives. My professional interests include medication management and psychotherapy. I enjoy assisting clients in becoming the best version of themselves and overcoming obstacles that may hinder their progress. I hold a Doctor of Nursing Practice from the University of Massachusetts Global and a Master's in Nursing from Walden University, which have equipped me with advanced knowledge and skills to provide high-quality mental health care.

As a PMHNP provider i provide comprehensive range of care that includes monitoring patient progress, assessing symptoms, educating patients on self-care techniques, managing medication regimens, offering psychotherapy or counseling sessions, and providing emotional support.

What is your typical process for working with clients?

As a PMHNP working with clients involves a comprehensive and holistic approach to mental health care. The process typically includes several key steps: 1. Initial Assessment: Conducting thorough psychiatric evaluations to gather information about the client's medical history, psychiatric symptoms, current medications, social and family history, and other relevant factors. Establishing rapport and building a therapeutic relationship with the client to facilitate trust and open communication. 2. Diagnosis and Treatment Planning: Formulating a differential diagnosis based on the assessment findings and diagnostic criteria outlined in the DSM-5 (Diagnostic and Statistical Manual of Mental Disorders, 5th Edition). Collaborating with the client to develop an individualized treatment plan that addresses their unique needs, preferences, and treatment goals. Discussing treatment options, including psychotherapy, medication management, lifestyle modifications, and other therapeutic interventions. 3. Medication Management: Prescribing and managing psychiatric medications, including psychotropic medications such as antidepressants, antipsychotics, anxiolytics, and mood stabilizers. Educating clients about the purpose, benefits, potential side effects, and risks associated with medications. Monitoring medication efficacy and safety through regular follow-up appointments and medication adjustments as needed. 4. Psychotherapy and Counseling: Providing evidence-based psychotherapy modalities such as cognitive-behavioral therapy (CBT), supportive therapy, and motivational interviewing. Conducting individual, and couples therapy sessions to address specific mental health concerns and improve coping skills. 5. Health Promotion and Education: Offering health education and promoting self-care practices related to mental health, stress management, relaxation techniques, sleep hygiene, nutrition, exercise, and substance use prevention.     Dr. Takha is very dedicated to her job and cares for each and every patient she recieves. Within 30 minutes of our appointment, she was able to figure out what was needed to handle my mental health issues. I would strongly recommend her for anyone that is nervous about finding mental health care.

    My review of my virtual appointments. The first initial appointment had to be rescheduled because I booked it online but the doctor was not available. The second appointment was on 30 minutes but I was not told until the time of my appointment that she had an emergency. So it was rushed. My third appointment again had to be rescheduled and the gentleman called me prior to my appointment but kept saying the wrong time of my original appointment and then booked me later in the day. No link was sent had to call and find out it was scheduled for a later time not the time he gave me earlier that day. Now I’m all about giving second chances and things happen. This last appointment. I got connected to the doctor right away and she tells me she has an appointment….and that she really wants to see me and she books me for a later appointment. Never got the link. No one answers the phone. I will not be back. If you see her I would say go in person because it’s not organized for virtual.
