Getting to know Smink Achila

Facing challenges like depression, anxiety, bipolar disorder, trauma, or mood disorders can feel overwhelming. I'm Smink Achila, a psychiatric nurse practitioner with over 16 years of experience helping individuals navigate their mental health journey. My passion for psychiatry stems from a desire to make a difference in a field often stigmatized and misunderstood. I believe in meeting you where you are, providing a nonjudgmental and empathetic space where you can openly share your experiences. Together, we'll work collaboratively to develop a personalized treatment plan that addresses your unique needs.

What should a new client know about working with you?

When you meet with me, you can expect a calm, judgment free space where your story comes first. I take time to listen to what you have been going through, what has helped or not helped, and what you hope will change. Together we talk through options in clear language and create a plan that fits real life, which may include medication, therapy strategies, or simple lifestyle adjustments. You are never expected to have everything “figured out” before you arrive. In our sessions you set the pace, your questions are always welcome, and every decision is made with you, not for you. My goal is that you leave each visit feeling heard, understood, and more hopeful about your next steps.

What is your typical process for working with clients?

A typical first visit is 60 minutes so we have time to really understand you, not just your symptoms. We’ll talk about what has been going on, your medical and mental health history, previous treatments, and what you want life to look like going forward. I may use screening tools, review medications or labs, and then walk you through options in clear language. Together we decide on a plan that can include supportive talk therapy, lifestyle changes, and medication when it’s likely to help. If we choose medication, prescriptions are sent quickly to your preferred pharmacy so you can get started without delay. Follow-up visits are usually 30 minutes and are part check-in, part therapy: we review how you’ve been feeling, any side effects, and what is or isn’t working in daily life, make room for supportive conversation and coping skills, and adjust the plan together so you leave each visit with clear next steps and a sense that you’re moving forward, not just “refilling” a prescription.
