Care Plans
Monthly membership plans designed for patients who want consistent, ongoing access to care. Each plan includes one telehealth visit per month, secure messaging for follow-up questions, lab review and medication refills, priority scheduling for evenings and weekends, and sick notes and basic paperwork. Additional visits are available at reduced member rates.
