About Dr. Yakov Simkhayev
Determination Physical Therapy was founded by Dr. Yakov Simkhayev, PT, DPT.
Dr. Simkhayev graduated from Touro College with a Doctorate Degree in Physical Therapy in 2005. Yakov’s Pre-Doctorate studies were completed at Hunter College of the City University of New York.
Prior to starting Determination Physical Therapy, Dr. Simkhayev has worked within the Long Island Jewish Health System at Franklin Hospital where he specialized in: acute, sub-acute and post-op patients. He has extensive experience in treating patients with diagnosis of CVA (commonly known as stroke), Multiple Sclerosis, and total knee and hip replacements.
Dr. Simkhayev’s clinical experience also extends to working in an outpatient setting with a focus on orthopedic and geriatric population. As a physical therapist, Dr. Simkhayev's experience is in treating numerous orthopedic and sports related injuries with an emphasis on using various manual therapy techniques.

I've had the pleasure of discovering that Yakov is very knowledgeable and experienced in the field of physical therapy. I have multiple conditions that I need physical therapy for, and he's able to treat all of them.
The office uses a variety of methods to treat an individuals condition and tailor it to you as opposed to cookie cutter therapy.
I'm pleasantly surprised and impressed by the attentiveness given by Yakov both on the treatment table and afterwards. He actually checked on me later in the day to see how I was doing! No one has ever cared enough or done that before, and I've had quite a few treat me.
Josh at the front desk has been there for a while and is very nice and helpful. Very easy to recommend you coming here.

Dr Yakov was really nice and friendly (but not obnoxious), listened to all of my concerns and took his time addressing the concerns and showing me how to stretch my legs effectively to alleviate my back pain. I never felt rushed by him and really appreciate the care! Highly recommended!

Dr. Simkhayev did an excellent job with me . I was having pain for 3 weeks and on medication and nothing was helping . I couldn't move my neck at all in any direction. The first day I had my appointment I walked out with motion in my neck and only in one day. I was extremely happy !! I will be going back to him for sure to continue my physical therapy. If I can give this Doctor more then 5 stars I will!! Definitely recommend. Thank you

Dr. Simkhayev is great. He listens well and takes his time incorporating the rehab. An issue I have is that there are lot of activity and distractions occuring and at times I feel my time can be better used.

Dr. Yakov is honestly the best. His bed side manner is great, he listens to you and makes you feel very comfortable. He wants nothing but for you to feel 100x better than you did when you first walk in. Wait time is not bad and the staff is very friendly. I highly do recommend him as he is probably one of the best doctors I have had.

I came to Determination Physical Therapy with immense pain in my right foot when I started. But, when I did my consultation with the Dr. Yakov, he was able to immediately determine what was wrong. And, right away was able to begin treatment the same day as my consultation. I left feeling much better because I was able to use my injured foot without as much restraint.
I have been doing therapy with Dr. Yakov for a little over a month and I can tell you that my injured foot is almost back to normal. Now, I am able to stand more than 5 minutes at a time and can walk for long periods of time, distances with little to no pain in my injured foot. I even got my mom to start seeing him for her injury-which was a feat in and of itself. She likes going to therapy now, too. Dr. Yakov knows what he is doing and explains your injury so that you can understand what is required. He is also very open to questions about your injury to help you understand.
FYI - I wanted to give a fair review so this review is 3 weeks post this appointment date and 7 weeks post my consultation.

I highly recommend Dr. Yakov. I first came to him in terrible pain after suffering a torn calf muscle and ruptured plantaris tendon in my knee. I couldn't walk at all or even put the slightest amount of weight on my leg even with crutches. Dr. Yakov calmly assured me it would get better and that I would be walking again without crutches in 1 month or less. This is exactly what happened. After seeing him 3 times a week for 3 weeks, I slowly began to walk without crutches, and after a month, I was nearly back to normal. Through his techniques of cupping, stretching excercises, and massage, among other things, he was able to pinpoint exactly where the structural problems were, and was able to correct and heal them over time.

Friendly and excellent staff. Front desk is always helpful and pleasant. Doctor Simkhayev is very knowledgeable when it comes to getting you fixed up. From setting up exercises at the vicinity to doing them at home my recovery has been quite fast. I have been doing rehab for a torn ACL as it's been a relaxing and stress free process. Determination makes sure everything is going smooth for me.

He is a fantastic physical therapist. He has a gentle, quiet manner which is very rare these days. He took the time to talk to me about what I was feeling and to understand my particular situation involving carpal tunnel. He worked on my wrists for almost 2 months and they feel much better. After a bad fall with several injuries, mostly knees and broken toes, Yakov Simkayev said: "I will make you better", and he did. His confidence helped me to regain my own confidence, as well as my former speed and agility.
Yakov is the rare combination of being very smart, polite, kind and compassionate. I would not hesitate to recommend Yakov and his team to anyone who needs the unique results of Physical Therapy.

May 13, 2016 was my first visit for PT, at this time I had an excruciating left shoulder and arm pain with very limited range of motion. So much that I could barely raise my arm for simple tasks as washing my hair, much less hold a hair blower.
Today June 14, I am able to lift, pull, hold with minimal or no pain.
Thank you Dr. Simkhayev for giving me back my arm.

Very thankful for his help. Very professional and knowledgeable! Everyone in the office is very helpful and friendly, making it a very inviting environment. Also, a huge plus is the fact that there is a parking lot in the rear.

Well where do I begin I must say he is an excellent physical therapist Dr.Yakov is very professional, friendly and attentive. In the past month I was suffering from back pain numbness from the waist to the legs and now the pain went away at least 50%. I would highly recommend him! Besides that he and his staff are very nice, caring, thoughtful and they really take the time to go through all the process. very comfortable place to be well taken care of. I give it 5stars!!! Dr. Yakov has been blessed with the best hands and he also has a heart of gold. He is very pleasant and patient and he tries to work with you as much as he can to ease the pain you are feeling.. Both me and my daughter are very happy with his treatment and will continue to go to him for full recovery. I believe I will be back to normal in no time. I highly recommend him to any one who has any need for a physical therapist take my advice go to him. :)
