SERVICES MANAGED BY KJCARES
1. Chronic conditions like hypertension, diabetes, medication refills, thyroid issues, depression, anxiety, insomnia, constipation, diarrhea, medication management.
2. Acute issues like cough, strep throat, home health orders, frequent falls, toothache, allergies, eye/skin/stomach infections.
3. Chronic Care Management—for patients with more than 2 chronic conditions.
4. Transition Care Management—for patients leaving hospital, outpatient surgical centers, rehab, or post-acute facilities.
5. **FOR PCP PATIENTS ONLY**—DME supplies, incontinence supplies, placard renewals, metrolift assessments.
6. For chronically debilitated patients unable to visit PCP in person, we provide in-home labs, X-rays, EKGs, ultrasounds, podiatry, and eye exams via mobile partners. **Must live in approved geographic area.**
**Be advised:**
- We do not prescribe/refill controlled meds like Adderall, Xanax, Ativan, Vyvanse, Ritalin, Focalin, Klonopin, hydrocodone, tramadol, codeine, Ambien, etc.
- We do not prescribe meds requiring prior authorizations.
- We prescribe Mounjaro, Wegovy, ZepBound, Ozempic ONLY if diabetic.
- We do not fill out FMLA, MVA, disability, or workplace/school accommodation forms.
- We issue lab/urine orders for LabCorp/Quest only. No MRI, CT, or imaging orders UNLESS you are a KJCARES PCP patient.

I hold a Master’s in Family Health Nursing from Texas Woman’s University and Bachelor’s degrees from the University of Texas at Austin and the University of Houston.

I scheduled an appointment at 10:30 am at 9:30 he changed my appointment to 11:30 when 11:30 came he never called and when I called his office was closed!! Don’t say you’re open to doing appointments if you are not available I do not recommend ZocDoc!!
