Getting to know Dr. Alejandro Comas

Hello — I’m Dr. Alex Comas, a board-certified Psychiatric-Mental Health Nurse Practitioner. I provide compassionate, evidence-based mental health care in a space where you can feel safe, seen, and respected exactly as you are. I specialize in working with LGBTQ+ individuals, transgender and gender-diverse patients, and those living with HIV, as well as adults navigating ADHD, depression, anxiety, and relationship challenges. My practice is built on the belief that mental health care should affirm your identity — not question it. As a doctoral-trained provider with full prescriptive authority, I offer comprehensive psychiatric evaluations, thoughtful medication management (including Schedule II–V when clinically appropriate), and integrated psychotherapy. I am also certified in Cognitive Behavioral Therapy (CBT), which allows us to work not only on symptom relief, but on long-term emotional resilience and personal growth. My approach blends medical precision with therapeutic compassion. I take the time to listen carefully, explain treatment options clearly, and collaborate with you to create a plan that feels aligned with your goals and values. Whether you’re seeking gender-affirming mental health support, navigating complex medication adjustments, or looking for structured therapy, we will move at a pace that feels safe and empowering. I offer both in-person and telehealth appointments and provide care in English and Spanish. You deserve care that honors your whole self — and I look forward to supporting you on your path toward healing and stability.

What should a new client know about working with you?

When you work with me, you can expect a collaborative and respectful experience. I don’t believe in rushed visits or one-size-fits-all treatment plans. Our first appointment is focused on understanding your story — not just your symptoms. I want to know what’s been working, what hasn’t, and what healing looks like for you. My style is warm, direct, and structured. I combine thorough psychiatric evaluation and medication management with evidence-based therapy, particularly Cognitive Behavioral Therapy (CBT). That means we will look at both the biological and psychological aspects of what you’re experiencing. If medication is appropriate, I explain options clearly, discuss risks and benefits, and involve you in every decision. For LGBTQ+, transgender, and gender-diverse clients, you can expect affirming, identity-respecting care without assumptions or judgment. Your lived experience matters here. I also believe in accountability and growth. Therapy with me is active — we set goals, track progress, and build practical tools you can use between sessions. My goal is not just symptom relief, but helping you develop confidence, stability, and long-term resilience. Above all, this is a space where you can show up fully as yourself.

What is your typical process for working with clients?

Our first appointment is a comprehensive psychiatric evaluation. We’ll review your current concerns, mental health history, medical background, past treatments, family history, and any social or identity factors that may be impacting your wellbeing. I may recommend lab work when appropriate to rule out medical contributors to symptoms. From there, we create a personalized treatment plan. If medication is indicated, I explain each option in detail — including benefits, side effects, and what to expect — so you feel informed and involved in the decision-making process. I manage the full spectrum of psychiatric medications and adjust thoughtfully over time based on your response. In addition to medication management, I practice structured, evidence-based therapy — particularly Cognitive Behavioral Therapy (CBT). That means sessions may include identifying patterns of thinking, learning coping tools, and developing strategies to improve mood, focus, or relationships. I believe combining therapy and medication (when appropriate) often leads to stronger and longer-lasting outcomes. Follow-up sessions typically focus on tracking progress, fine-tuning medications if needed, building skills, and addressing new challenges as they arise. My approach is active and collaborative — we set goals, measure progress, and adjust together. Whether you’re seeking support for ADHD, mood concerns, anxiety, gender-affirming care, or complex medication management, my goal is to provide structured, affirming, and individualized care that supports long-term stability and growth.

We work with many major insurance plans, and our team is happy to help verify your benefits prior to your appointment so you understand your coverage and expected costs. Copays, deductibles, and out-of-pocket expenses vary by plan, and we strive to provide transparency upfront. For individuals who prefer not to use insurance or who have out-of-network benefits, we offer competitive self-pay options designed to keep care accessible. We believe high-quality mental health treatment should remain within reach. In addition to traditional psychiatric services and therapy, we also offer alternative and interventional treatments when clinically appropriate. Transcranial Magnetic Stimulation (TMS) therapy is available at $200 per session for self-pay patients. The total number of sessions depends on the protocol used and your specific diagnosis. We also provide intramuscular (IM) ketamine treatment at $450 per session, with a typical course consisting of 6–8 sessions depending on your individualized plan of care. All treatment recommendations are based on a comprehensive psychiatric evaluation and medical necessity. Our goal is to create a clear, personalized plan that aligns with both your clinical needs and financial considerations. If you have questions about coverage or payment options, our team is happy to walk you through the process before you begin care.
