Getting to know Dr. Alexander Morrill

Hello and welcome! If you are looking for a psychiatrist—or perhaps considering switching from the psychiatrist you are currently seeing—the chances are good that you have been asking yourself some difficult questions. For example, you may be wondering: * Should I be taking psychiatric medications? Are there other strategies available for managing my symptoms and reaching my goals? * If I am going to take psychiatric medications, which ones should I take (and at what dosage)? How risky are they? Am I on too many medications? How many medications are "too many"? * Should I be seeing a talk therapist? If so, what kind? * Are there lifestyle changes that might help me feel better? * What about all these "alternative medicine" treatments I’ve read about? Do they work? Are they safe? * What should I know about newer FDA-approved treatments such as transcranial magnetic stimulation (TMS) and ketamine (Spravato)? My primary task as a psychiatrist is to help you find answers to these questions. I do not aim to "dictate" a course of treatment but to advise you on a range of possible approaches, describing the risks and benefits of each. I strive to do this in a manner that is clear, detailed, easily understood, and—most important of all—unbiased. (I do not accept speaking fees, free steak dinners, or other forms of payment from drug companies.) I earned my Doctor of Medicine degree from the Sackler School of Medicine at Tel Aviv University, which has shaped my commitment to evidence-based, patient-centered care.

What should a new client know about working with you?

Before deciding to book an appointment, you should be aware of my basic policies regarding controlled or addictive medications. PART 1 OF 2: BENZODIAZEPINES "Benzodiazepines" are a class of addictive medications often used for anxiety or sleep (e.g. Klonopin, Xanax, Ativan, Valium, and Restoril). As a general rule, I do not prescribe these medications on a long-term basis. I do not feel that these medications are safe or effective as a long-term treatment. (I do sometimes prescribe these medications for occasional use, e.g., during plane flights). If you are currently taking one or more of these medications, and are interested in stopping, I can help. Over the years, I have helped many people successfully taper off benzodiazepines, in a manner designed to minimize any withdrawal effects. In most cases I have noticed that the patient experiences improvement of symptoms and greater mood stability after these medications have been stopped. PART 2 OF 2: ADHD MEDICATIONS (A.K.A. "STIMULANTS") Although I do sometimes prescribe stimulants, please be aware that I prescribe them very selectively. Like many of my colleagues, I believe that these medications are currently over-prescribed. In the past 25 years, the use of stimulant medication in the United States has increased by over 500%. If you are a prospective patient who is concerned about issues with focus or attention, you should be aware of the following: (1) The diagnostic criteria for ADHD are subjective. Psychiatrists often disagree with each other about whether a specific patient meets criteria for ADHD. Thus, the fact that you have been diagnosed with ADHD by another physician does not guarantee that I will arrive at the same diagnosis. (2) Problems with attention/focus are sometimes caused, in whole or in part, by clinical issues other than ADHD (e.g. sleep issues, emotional issues, etc). My goal as a physician is always to address these issues before considering treatment with stimulants.

What is your typical process for working with clients?

Treatment with me begins with a 60-minute introductory visit. Followups are typically 20 minutes each, but it is not uncommon for me to schedule 30-minute followups when needed. The extra time does not affect how the session is billed, and I find that having the extra time is often helpful for both myself and the patient. The frequency of follow-up visits is determined by what makes sense clinically, and by your personal preference. I have patients who only need to see me a few times a year. Other patients may wish to meet as often as every two weeks. If an unexpected issue should come up between appointments, I am generally able to get established patients an appointment with me within a few days. Although my main focus is on medications, I do have extensive training and experience in the techniques of talk therapy. In addition to asking you about symptoms and side effects, I will be asking you questions about your relationships, work, and life goals. If you have (or are planning to acquire) an outside therapist, I will be happy to consult with that therapist as needed in order to develop a holistic and comprehensive treatment plan. When needed, I will also consult with your primary care physician in order to address any physical issues that may be affecting your mental well-being .

    Dr. Morrill has an arrogant and uncurious demeanor. He seems to think that his external (biased) perceptions of you outweigh your own descriptions of your experince. I am a self diagnosed ADHD/ASD person and when I brought it up to him he did not even ask any questions other than ones that he felt disproved my assessment. He is very ableist and biased in his thinking and his perception of what neurodivergence looks like. I am a white AMAB person and he didn't even believe me; I can't imagine what it is like to see him as a person of color and/or AFAB person. If you are not a white cis neurotypical man stay far away. He will not help you or even believe what you say. Not queer friendly. Not autism friendly. Not ADHD friendly. He seemed to think I was faking it to get pills because he didn't 'see' my neurodivergence. ??

Upon his suggestion I was diagnosed with a sleeping disorder I didn’t even know I had. I’d say he is conservative with medication changes and tries to balance the side effects with alternative ways in which symptoms can be addressed.

    I would recommend Dr Alexander to anyone who needs a knowledgeable psychiatrist who will is honest and straight-forward. My evaluation took only an hour, compared to my expectation of 2-3 hours. However, it was not the best fit for me as I was looking for a second opinion for my ADHD while he was honest that he rarely provides ADHD diagnoses unless for extreme cases. I did not receive any mental health evaluation forms at any point, which I think would've helped our discussion better. Even though, I am managing my possible ADHD symptoms and have overlapping anxiety symptoms, it shouldn't dismiss the mental disorder/illness since many are on a spectrum. He did offer some insight for my anxiety, medication management, and inconclusive opinion that I may have ADHD based on my primary care's diagnosis and medication.

    Dr. Morrill was skeptical of my diagnosis from a different doctor in a way that almost felt dismissive. However, he was willing to work with me as long as I would be willing to work with him. He doesn’t want to medicate anyone who he’s not absolutely certain needs medication.

    Dr. Morrill comes across always annoyed, he expects the patient to know the medications/dosage they are on and he never fills the prescriptions properly or in a timely manner. When refilling, there is always a weeks wait for approval.
