We are in-network with select insurance plans and also welcome self-pay patients. Insurance Coverage Coverage varies depending on your specific plan (HMO, PPO, POS, EPO), deductible status, and behavioral health benefits. We recommend contacting your insurance carrier to confirm your copay, coinsurance, deductible, telehealth coverage, and whether prior authorization is required. As a courtesy, we verify benefits prior to your appointment when possible; however, patients are ultimately responsible for understanding their individual insurance plan and any associated costs. Copays & Deductibles Copays, coinsurance, or deductible balances are due at the time of service. If you have a high-deductible plan, you may be responsible for the contracted rate until your deductible has been met. Out-of-Network Benefits If we are not in-network with your plan, you may still qualify for reimbursement through out-of-network benefits. We can provide a superbill upon request for you to submit to your insurance company. Self-Pay Options Transparent self-pay rates are available for patients without insurance or those who prefer not to use insurance. Please contact our office for current fees.
